基于信号通路的中药治疗膜性肾病的机制研究概述

膜性肾病是一种自身免疫性疾病,是引起终末期肾脏病的主要原因,目前治疗存在许多争议.近年来,中药治疗膜性肾病引起广大研究者的注意,大量研究证实中药可以降低炎症反应、抗氧化应激、抑制足细胞凋亡、诱导足细胞自噬,其主要通过调控磷脂酰肌醇3-激酶/蛋白激酶B(phosphatidyl inositol 3-kinase/protein kinase B,PI3K/Akt)、转化 生长因 子-β1/Smad 蛋 白(transforming growth factor-β1/Smad protein,TGF-β1/Smad)、还原型烟酰胺腺嘌呤二核苷酸磷酸氧化酶4(reduced nicotinamide adenine dinucleotide phosphate oxidase 4,NOX4)、核因子 E2 相关因子2/血红素氧合酶-1(nuclear factor E2 related factor 2/heme oxygenase-1,Nrf2/HO-1)、丝裂原活化蛋白激酶(mitogen activated protein kinase,MAPK)、胞外信号调节激酶/胞浆型磷脂酶 A2(extracellular signal-regulated kinase/cytoplasmic phospholipase A2 ERK/cPLA2)、雷帕霉素靶蛋白(target protein of rapamycin,mTOR)、核转录因子-κB(nuclear transcription factor-κB,NF-κB)与足细胞凋亡相关信号通路等多条信号通路来实现.本文通过总结中药调控相关信号通路治疗膜性肾病的机制研究,旨在为后续实验研究开拓思路,为临床应用提供科学依据.
Review on the mechanism of Chinese medicine treatment of membranous nephropathy based on signaling pathway
Membranous nephropathy is an autoimmune disease and the main cause of end-stage renal disease.In recent years,the treatment of membranous nephropathy by Chinese medicine has attracted attention of many researchers.A large number of studies have confirmed that Chinese medicine can reduce inflammation,resist oxidative stress,inhibit podocyte apoptosis and induce podocyte autophagy.It is mainly through the regulation of phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt),transforming growth factor-β1/Smad protein(TGF-β1/Smad),reduced nicotinamide adenine dinucleotide phosphate oxidase 4(NOX4),nuclear factor E2 related factor 2/heme oxygenase-1(Nrf2/HO-1),Mitogen-activated protein kinase(MAPK),extracellular signal-regulated kinase/cytoplasmic phospholipase A2(ERK/cPLA2),rapamycin target protein(mTOR),nuclear transcription factor-κB(NF-κB)and podocyte apoptosis related signaling pathway and other signal pathways to achieve.In this paper,we summarized the mechanism of Chinese medicine regulating related signaling pathways in the treatment of membranous ne-phropathy,aiming to develop ideas for subsequent experimental research and provide scientific basis for clinical application.
