清瘟败毒饮对脓毒症肺损伤大鼠的治疗作用及对铁死亡的影响

目的 研究清瘟败毒饮对肓肠结扎穿孔术(cecal ligation and puncture,CLP)脓毒症肺损伤大鼠的治疗作用及机制研究.方法 取75只SD健康雄性大鼠,按随机数字表法分为假手术组,模型组,清瘟败毒饮低、中、高剂量组,共5组.假手术组只切开腹壁并缝合,不进行CLP,灌胃等体积生理盐水,其余各组运用CLP建立脓毒症肺损伤大鼠模型.检测治疗后各组大鼠生存率、肺组织的湿干重比、肺泡灌洗液(bronchoalveolar lavage fluid,BALF)总蛋白含量、BALF总细胞数、肺组织脂质过氧化水平及肺组织HE染色,研究清瘟败毒饮对脓毒症肺损伤的治疗作用.运用Western-Blot法,检测各组大鼠肺组织中铁蛋白重链(ferritin heavy chain,FTH)、铁蛋白轻链(ferritin light chain,FTL)、转铁蛋白(transferrin)、谷胱甘肽过氧化酶 4(glutathione peroxidase 4,GPX4)蛋白表达情况,探讨清瘟败毒饮对脓毒症肺损伤大鼠模型中铁死亡相关因子的影响,初步研究其作用机制.结果 模型组生存率为40.00%,清瘟败毒饮低、中、高剂量组生存率分别为53.33%、60.00%、73.33%;同假手术组相比,模型组能明显增加脓毒症肺损伤大鼠肺湿干重比(W/D)(P<0.01),模型组BALF上清液总蛋白含量显著增多(P<0.01),总细胞数显著增多(P<0.01).同模型组相比,清瘟败毒饮中、高剂量组能减少脓毒症肺损伤大鼠肺W/D(P<0.05,P<0.01),清瘟败毒饮低、中、高剂量组均能显著减少BALF上清液总蛋白含量(P<0.05,P<0.01),减少总细胞数(P<0.05);同假手术组相比,模型组大鼠白细胞介素-6(interleukin-6,IL-6)、白细胞介素-1 β(interleukin-1 β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、丙二醛(malondialdehyde,MDA)、4-羟基壬烯醛(4-hydroxide,4-HNE)、总谷胱甘肽(total-glutathione,T-GSH)、氧化型谷胱甘肽(oxidized glutathione,GSSG)和二价铁离子(Fe2+)水平显著升高(P<0.01).同模型组相比,清瘟败毒饮中剂量和高剂量组大鼠IL-6、IL-1β和TNF-α水平显著降低(P<0.01),清瘟败毒饮高剂量组大鼠MDA和4-HNE水平显著降低(P<0.01);清瘟败毒饮各剂量组均能降低GSSG和Fe2+水平,尤其以清瘟败毒饮高剂量组更为显著(P<0.01).同假手术组相比,模型组大鼠中T-GSH、GSH水平和GSH/GSSG比值明显降低,清瘟败毒饮各剂量组均能显著提高T-GSH、GSH水平和GSH/GSSG比值(P<0.01),模型组大鼠FTH、FTL、Transferrin蛋白表达量显著增加(P<0.01),GPX4蛋白表达明显下降(P<0.01).同模型组相比,经各剂量清瘟败毒饮干预后FTH、FTL的蛋白表达量显著降低(P<0.01),而中、高剂量清瘟败毒饮干预同样可以降低Transferrin的蛋白表达量(P<0.01);此外,清瘟败毒饮各剂量组可明显升高GPX4蛋白表达(P<0.01).结论 清瘟败毒饮对脓毒症肺损伤大鼠的治疗作用明确,其治疗机制可能同抑制铁死亡相关.
The therapeutic effect of Qingwen Baidu Decoction sepsis-induced lung injury in rats and its influence on ferroptosis
Objective To investigate the therapeutic effect and mechanism of Qingwen Baidu Decoction on sepsis-induced lung injury in rats with cecal ligation and puncture(CLP).Methods A total of 75 healthy male SD rats were randomly divided into sham operation group,model group,and low,medium,and high dose Qingwen Baidu Decoction groups,with 30 rats in each group.The sham operation group only underwent laparotomy and suture without CLP and was orally administered with an equal volume of normal saline.The remaining groups underwent CLP to establish a sepsis-induced lung injury rat model.The therapeutic effect of Qingwen Baidu Decoction on sepsis-induced lung injury was investigated by detecting the survival rate,wet-to-dry weight ratio of lung tissue,total protein content and total cell count in bronchoalveolar lavage fluid(BALF),lung tissue lipid peroxidation level,and HE staining after treatment.In addition,the method of western blot was used to detect the expression of FTH,FTL,transferrin,and GPX4 proteins in lung tissue of each group of rats,to explore the effect of Qingwen Baidu Decoction on iron death-related factors in sepsis-induced lung injury rat model,and to preliminarily study its mechanism of action.Results Qingwen Baidu Decoction intervention could effectively reduce the lung W/D ratio,total protein content,and total cell count in BALF of sepsis-induced lung injury rats.It could also decrease the levels of IL-6,IL-1 β,TNF-α,MDA,and 4-HNE in sepsis-induced lung injury rats,reduce the levels of GSSG,and Fe2+,increase the level of T-GSH,GSH,and improve the GSH/GSSG ratio.Moreover,Qingwen Baidu Decoction could reduce lung tissue pathological changes such as alveolar rupture or fusion and inflammatory cell infiltration caused by sepsis-induced lung injury.In addition,Qingwen Baidu Decoction could reduce the protein expression levels of iron death-related factors FTH,FTL and transferrin,while increase the protein expression level of GPX4.Conclusion Qingwen Baidu Decoction has a clear therapeutic effect on sepsis-induced lung injury in rats,and its therapeutic mechanism may be related to the inhibition of iron death.
