针刺治疗慢性荨麻疹机制研究进展

临床研究证明针刺治疗慢性荨麻疹(chronic urticaria,CU)疗效肯定,但其具体的作用机制仍未明确.目前的研究发现了针刺的起效机制是通过多途径完成的,如针刺通过降低血清免疫球蛋白E(immunoglobulin E,IgE)及其他细胞因子、化学介质水平,升高CD4+/CD8+水平,调节辅助性T细胞(helper T cell,Th)1/Th2失衡、调节Th17/调节性T细胞(regular T cell,Treg)水平来调节免疫功能.肥大细胞是荨麻疹的主要效应细胞,针刺通过调控多信号通路,在肥大细胞活化的情况下,抑制了肥大细胞的脱颗粒过程,减少炎性介质的释放,针刺还可能通过修饰表观遗传基因表达、改善脑功能以缓解慢性荨麻疹症状.虽然慢性荨麻疹发病机制以及针刺的起效机制的研究大多集中于免疫机制,检测指标也多为下游的细胞及细胞因子,但是荨麻疹的中枢病理机制也同样值得重视.本文对近10年针刺治疗荨麻疹机制研究的文献进行整理和归纳,以期为揭示针刺治疗荨麻疹的作用机制提供证据,为临床提供理论依据.
Research progress on mechanism of acupuncture treatment of chronic urticaria
Clinical studies have shown that acupuncture treatment for chronic urticaria(CU)has a definite therapeutic effect,but its specific mechanism of action is still unclear.Current research has found that the mechanism of action of acupuncture is achieved through multiple pathways,such as reducing serum immunoglobulin E(IgE)and other cytokines and chemical mediators,increasing CD4+/CD8+levels,regulating helper T cell(Th)1/Th2 imbalance,and regulating Th 17/regulatory T cell(Treg)levels to affect immune function.Mast cells are the main effector cells of urticaria.Acupuncture regulates multiple signaling pathways and,under the activation of mast cells,inhibits the degranulation process of mast cells,reducing the release of inflammatory mediators.Acupuncture may also alleviate chronic urticaria symptoms by modifying epigenetic gene expression and improving brain function.Although the research on the pathogenesis of chronic urticaria and the effective mechanism of acupuncture mostly focuses on the immune mechanism,and the detection indicators are mostly downstream cells and cytokines,the central pathological mechanism of urticaria also deserves attention.The literature on the mechanism of acupuncture treatment for urticaria was summarized and organized in this article in the past 10 years,in order to provide evidence for revealing the mechanism of acupuncture treatment for urticaria and provide theoretical basis for clinical practice.
