基于数据挖掘探讨董建华院士治疗慢性胃炎的用药规律

摘要
目的 基于数据挖掘探讨董建华院士治疗慢性胃炎的用药规律.方法 通过收集董建华院士于北京中医药大学东直门医院工作期间的相关医案,以及已公开发表的董建华院士论著中的代表性医案,纳入慢性胃炎的相关病案处方信息,运用中医传承计算平台进行频数、四气五味及归经分析,运用SPSS Modeler 18.0 软件进行关联规则分析,以及运用SPSS Statistics 26.0 软件进行聚类分析和因子分析.结果 共纳入192 首处方,得到 12 个高频证型、10 个高频治则;涉及 183味中药,使用频次较高的中药为陈皮、香附、枳壳、川楝子、香橼,常用药物功效为理气类、补虚类、清热类及化痰类;药性以寒、温为主,药味以苦、辛为主,归经以脾、胃、肺、肝经为主;关联规则分析中支持度最高的二味药物为延胡索—川楝子,三味药物为焦麦芽—焦山楂—焦神曲;聚类分析得到 4个核心处方,因子分析共提取出8 个公因子.结论 董建华院士治疗慢性胃炎遵循"理法相融、方药相对"的原则,用药以轻灵流畅见长.
Abstract
Objective Based on data mining,this study explores the medication rules of academician Dong Jianhua in the treatment of chronic gastritis.Methods The medical records of academician Dong Jianhua during his tenure at Dongzhimen Hospital of Beijing University of Chinese Medicine and his representative published works were collected,and the prescription information of relevant medical records of chronic gastritis was included.Statistical analysis was carried out based on the frequency analysis,association rule analysis and cluster analysis of traditional Chinese medicine inheritance computing platform,and association rule analysis was performed using SPSS Modeler 18.0 software,as well as cluster analysis and factor analysis using SPSS Statistics 26.0 software.Results A total of 192 prescriptions were included in this study,obtaining 12 high-frequency evidence types and 10 high-frequency treatment rules.A total of 183 Chinese herbs were involved,and the core Chinese herbs were dried tangerine peel,nutgrass galingale rhizome,orange fruit,szechwan chinaberry fruit and citron fruit.Common drug effects were regulating qi,tonifying deficiency,clearing heat and resolving phlegm;the medicinal properties were mainly cold and warm,and the medicinal tastes was mainly bitter and spicy;the meridians were mainly spleen,stomach,lung,and liver.The two drugs with the highest support in the association rule analysis were rhizoma corydalis-fructus toosendan,and the three drugs were caramel malt-parched hawthorn fruit-medicated leaven;4 core prescriptions were obtained using cluster analysis,and 8 common factors were extracted using factor analysis.Conclusion In the treatment of chronic gastritis,academician Dong Jianhua adheres to the principle of integration of rationale and method,and correspondence of prescription and medicine,and is characterized by lightness and smoothness in the use of medicine.
