丹参及其有效成分治疗糖尿病肾病的作用机制研究进展

丹参中含有丹参素、丹酚酸、丹参酮等多种有效成分,对糖尿病肾病的各种病理变化均具有调控作用.丹参中诸多有效成分可以通过调节糖代谢以及改善胰岛素分泌和抵抗来调控血糖水平;通过调节多种细胞因子改善血脂水平、调节脂质代谢,及调节血流灌注、抑制血栓形成改善肾脏血流,可以作用于多个靶点来减少肾脏的氧化应激损伤、提高抗氧化能力;通过抑制转化生长因子-β(transforming growth factor-β,TGF-β)-1 和核因子 κB(nuclear factor kappa-B,NF-κB)的表达以及调节磷脂酰肌醇3-激酶/蛋白激酶B信号通路减轻炎症反应,还可以通过调控TGF-β/Smad信号通路、乙酰肝素酶/多配体蛋白聚糖-1信号通路、Wnt/β-catenin信号通路、去乙酰化酶1信号通路、NF-κB信号通路、Notch信号通路以及Janus激酶/信号转导及激活转录因子信号通路来发挥抗肾脏纤维化的作用,从而减轻糖尿病肾病患者的肾脏损伤,发挥对肾脏的保护作用.本文参考近年来有关丹参及其有效成分干预糖尿病肾病的相关研究,总结了丹参及其有效成分对糖尿病肾病诸多病理变化的治疗效果,对丹参及其有效成分在糖尿病肾病发生发展过程中发挥积极影响的作用机制进行了归纳,为临床应用丹参治疗糖尿病肾病提供科学依据.
Research progress on the mechanism of salvia miltiorrhiza and its effective ingredients in the treatment of diabetic nephropathy
Salvia miltiorrhiza contains tanshinol,salvianolic acid,tanshinone and other effective ingredients,which can regulate various pathological changes of diabetic nephropathy.Many effective ingredients in Salvia miltiorrhiza can regulate blood glucose levels by regulating glucose metabolism and im-proving insulin secretion and resistance,improving blood lipid levels and regulating lipid metabolism by regulating multiple cytokines,improving renal blood flow by regulating blood flow perfusion and inhibiting thrombosis,it can act on multiple targets to reduce oxidative stress damage to the kidneys and improve an-tioxidant capacity,reducing inflammatory response by inhibiting the expression of TGF-β1 and NF-κB and regulating the PI3K/Akt signaling pathway,it can also be achieved by regulating TGF-β/Smad,HPSE/SDC1,Wnt/β-Catenin,Sirt1,NF-κB,Notch,and JAK/STAT signaling pathway to exert anti fibrotic effects,thus reducing renal damage in patients with diabetic nephropathy and playing a role in renal protection.This article reviewed the relevant research on the intervention of salvia miltiorrhiza and its effective ingredients in diabetic nephropathy in recent years,summarized the therapeutic effect of salvia miltiorrhiza and its effective ingredients on many pathological changes of diabetic nephropathy,and summarized the mechanism of action of salvia miltiorrhiza and its effective ingredients in the occurrence and development of diabetic nephropathy,providing scientific basis for clinical application of salvia miltiorrhiza in the treatment of diabetic nephropathy.
