基于TOLL样受体4/核因子-κB通路探讨针刺对原发性高血压大鼠肾脏巨噬细胞极化调节和肾保护作用

目的 探讨捻转补泻手法对原发性高血压大鼠(spontaneous hypertension rat,SHR)血压和肾脏中巨噬细胞极化的影响.方法 实验选用12只雄性Wistar-Kyoto(WKY)大鼠作为空白对照组,36只雄性SHR随机分为模型组、捻转泻法组、捻转补法组,每组12只.捻转补法组与捻转泻法组分别在双侧太冲穴进行针刺捻转泻法和针刺捻转补法,每天1次,共两周,中间休息1次.空白对照组与模型组进行相同时长与频次的抓捉固定,不做针刺干预.在干预期间使用尾压法测量大鼠血压,两周后收集大鼠血清和肾组织,采用ELISA法观察血清中去甲肾上腺素(norepinephrine,NE)、血管紧张素Ⅱ(Angiotensin Ⅱ,Ang Ⅱ)含量和肾脏中白细胞介素(interleukin,IL)-1 β、IL-10含量.Western blot 法观察肾脏中 Toll 样受体 4(Toll-like receptor 4,TLR4)、核转录因子-κB(nuclear factor-KB,NF-κB)、分化簇(cluster of differentiation,CD)86、CD206 蛋白的表达情况.用苏木素—伊红(hemotoxylin-eosin,HE)染色观察肾组织形态.结果 (1)与空白对照组相比,模型组大鼠血压显著升高(P<0.01),血清NE、AngⅡ浓度显著上升(P<0.01),肾脏中TLR4、NF-κB、CD86、IL-1β含量显著上升(P<0.01),CD206、IL-10含量显著下降(P<0.01).HE染色结果显示,SHR出现肾小管与肾小球形态不规则,存在炎性细胞浸润.(2)与模型组相比,捻转泻法组和捻转补法组血压显著降低(P<0.01),血清NE、AngⅡ浓度显著降低(P<0.01),肾中TLR4、NF-κB、CD86表达显著下降(P<0.01),IL-1β 浓度降低(P<0.05,P<0.01),CD206、IL-10 含量显著上升(P<0.01),肾小管与肾小球形态正常,存在少量炎性细胞浸润.(3)与捻转泻法组相比,捻转补法组血压较高(P<0.05),血清中NE、Ang Ⅱ浓度和肾脏中TLR4、NF-κB、CD86、CD206、IL-1β、IL-10均存在差异,且差异具有统计学意义(P<0.05,P<0.01).结论 针刺太冲穴并行捻转补泻手法通过降低交感神经兴奋性降低AngⅡ含量,抑制TLR4/NF-κB通路调节巨噬细胞极化来抑制IL-iβ,增加IL-10浓度,起到降压和肾保护作用,且捻转泻法的效果优于捻转补法.
Modulation of macrophage polarization and nephroprotective effects of acupuncture in SHR kidneys explored based on the TLR4/NF-κB pathway
Objective To observe the effects of acupuncture manipulation on blood pressure and renal inflammatory environment in spontaneously hypertensive rats(SHR),and to elucidate the mechanism of antihypertensive and nephroprotective effects of the manipulation.Methods 36 male SHRs were randomly divided into 3 groups including the model group(SHR group),the twirling reducing manipulation group(TRD group)and the twirling reinforcing manipulation group(TRF group).In addition,12 WKY rats were used as the control group(WKY group).The TRD and the TRF groups were subjected to twirling reducing manipulation and twirling reinforcing manipulation in LR3 respectively,once a day for two weeks,with one day of rest in between the two weeks.During the intervention period,we measured the blood pressure of the rats using the tail pressure method.After two weeks,the serum and kidney tissues of rats in each group were collected,and the concentrations of norepinephrine(NE)and Angiotensin Ⅱ(AngⅡ)in serum and the contents of interleukin(IL)-1 β and IL-10 in kidneys were detected by Elisa method;the expressions of Toll-like receptor 4(TLR4),nuclear factor-κB(NF-κB),cluster of differentiation(CD)86 and CD206 proteins in kidneys were detected by Western blot method;and morphological changes of kidneys were observed by HE staining.Results(1)Compared with the WKY group,the blood pressure of rats in the SHR group was significantly increased(P<0.01),serum NE and AngⅡ concentrations were significantly increased(P<0.01),and the content of TLR4,NF-κB,CD86,IL-1 β in kidney was significantly increased(P<0.01)and the content of CD206 and IL-10 was significantly decreased(P<0.01).The results of HE staining showed that SHRs appeared irregular renal tubular and glomerular morphology,with the presence of inflammatory cell infiltration.(2)Compared with SHR group,blood pressure was significantly decreased in the TRD group and the TRF group(P<0.01),serum NE and AngⅡ concentrations were significantly decreased(P<0.01).The expression of TLR4,NF-κB,CD86 the kidney was significantly decreased(P<0.01),and the concentration of IL-1 β was decreased(P<0.05,P<0.01),and the content of CD206 and IL-10 was significantly increased(P<0.01).The morphology of renal tubules and glomeruli was normal,and a small amount of inflammatory cell infiltration was present.(3)Compared with the TRD group,the TRF group had higher blood pressure(P<0.05),and changes in serum concentrations of NE,AngⅡ,and renal levels of TLR4,NF-κB,CD86,CD206,IL-1 β,and IL-10,with statistically significant differences(P<0.05 or P<0.01).Conclusion Acupuncture at LR3 with twirling reinforcing and reducing manipulation inhibited renal inflammation by decreasing sympathetic excitability to lower AngⅡ and inhibiting TLR4/NF-κB pathway to regulate macrophage polarization to exert both antihypertensive and nephroprotective effects,and the twirling reducing manipulation was more effective than the twirling reinforcing manipulation.
