摘要
目的 检索、筛选并汇总成人脑卒中患者久坐行为(SB)管理的最佳证据.方法 依据"6S"金字塔证据资源模型,系统检索、筛选国内外中英文数据库中有关成人脑卒中患者SB管理的高级别证据,包括临床决策、推荐实践、指南、证据总结、专家共识、系统评价、随机对照实验,检索时限为建库至2022年12月30日,对文献进行独立质量评价,并对提取整合的证据进行等级划分.结果 共纳入15篇文献,其中包括1篇临床决策、4篇指南、1篇推荐实践、2篇专家共识、4篇系统评价、3篇随机对照实验,从目标设定、多学科团队协作、干预时机、SB监测、SB提示、运动锻炼、健康教育及随访7个维度汇总22条证据.结论 形成的脑卒中患者的SB管理的证据总结较为系统、完善,以期为制定科学、规范的脑卒中患者SB管理方案提供循证依据,有利于改善患者预后.
Abstract
Objective To retrieve,screen,and summarize the best evidence on the management of sedentary be-haviour(SB)in adult stroke patients,and to provide an evidence-based basis for clinical nursing practice.Methods Based on the"6S"pyramid evidence resource model,high-level evidence on the management of SB in adult stroke pa-tients was systematically searched and screened in Chinese and English databases at home and abroad,including clin-ical decision-making,recommended practices,guidelines,evidence summaries,expert consensus,systematic re-views,and randomized controlled trials.The search time limit was from the establishment of the database to Decem-ber 30,2022.The quality of literature was independently assessed and the extracted and integrated evidence was gra-ded.Results A total of 15 papers were included,including 1 clinical decision,4 guidelines,1 recommended practice,2 expert consensuses,4 systematic reviews,and 3 randomized controlled trials,and 22 pieces of evidence were sum-marized from 7 dimensions:goal setting,multidisciplinary teamwork,timing of intervention,SB monitoring,SB prompts,exercise and movement,health education,and follow-up.Conclusion The evidence summary of SB man-agement in stroke patients is systematic and perfect,which is expected to provide an evidence-based basis for the de-velopment of a scientific and standardized SB management program for stroke patients,which is conducive to the im-provement of patient prognosis.
