Investigation of family dynamics and decision dilemma in influencing factors of delayed medical treatment in patients with first-episode acute myocardial infarction
Objective To explore the factors leading to delayed medical treatment in first-episode acute myocardial infarction(AMI)patients,with a particular focus on the impact of family dynamics and decision-making difficulties on delayed medical treatment.Methods A total of 232 initial AMI patients who were treated in a Class Ⅲ Grade A hospital in Henan province from February to August 2022 were selected by using convenience sampling method,and categorized into the delayed medical care group and the prompt medical care group.General information,systematic family dynamics,and the severity of decision dilemmas were investigated and analyzed by questionnaire.Results A-mong 232 patients with first-episode AMI,126 persons experienced delayed medical treatment,accounting for 54.31%.There were differences in the distribution of variables such as educational level,residence,monthly in-come,drinking history,history of diabetes,and main clinical symptoms of patients with first-episode AMI in terms of medical delay(P<0.05).The results of logistic regression analysis showed that education,residence,monthly income,drinking history,history of diabetes,main clinical symptoms,family dynamics,and decision-making dilem-ma were the independent influencing factors for the delay of medical treatment in patients with first-episode AMI P<0.05).The family dynamics score of the first-episode AMI patients in the delayed medical treatment group was(94.75±6.35)points,and higher than that in the timely medical treatment group with(91.16±8.27)points,and the difference was statistically significant(P<0.05);The score of decision-making dilemma in the delayed medical treatment group was(55.19±5.39)points,higher than that in the timely medical treatment group with(51.03± 6.88)points,and the difference was statistically significant(P<0.05).Among them,family dynamics and deci-sion-making difficulties had a positive impact on delayed medical treatment in first-episode AMI patients(ORfamily=1.054,OR difficulties=1.134).Conclusion The medical care delay of patients with first-episode AMI is at a high level.Low education,living in rural areas,low monthly income,drinking history,history of diabetes,symptoms other than chest pain,poor family dynamics,and decision-making difficulties are more prominent in the first-episode AMI patients with medical delay.It has important value to strengthen health education,improve patients'family dynam-ics,and reduce decision-making difficulties in improving the delaying medical treatment for patients with first-episode AMI.
