摘要
目的 检索、评价并整合支气管扩张症患者气道廓清技术(ACT)的最佳证据.方法 采用PIPOST模式构建循证问题,按照"6S"金字塔证据模型,系统检索国内外数据库及专业协会网站中关于支气管扩张症患者ACT的指南、临床实践、专家共识、证据总结和系统评价,检索时限为建库至2023年5月1日.结果 共纳入14篇文献,包括指南5篇、临床实践2篇、专家共识2篇和系统评价5篇.从评估、干预措施、时间和频率、注意事项、自我管理5个方面,共总结出21条最佳证据.结论 医护人员可基于我国国情,考虑患者现状、结合实际临床情境及支气管扩张症患者ACT的最佳证据,制定支气管扩张症患者ACT方案,改善其临床结局,提高生活质量.
Abstract
Objective To retrieve,evaluate and integrate the best evidence of airway clearance techniques(ACT)in patients with bronchiectasis.Methods Using PIPOST mode to construct evidence-based questions,according to the"6S"pyramid evidence model,guidelines,clinical practice,expert consensus,evidence summary,and systematic review on ACT in patients with bronchiectasis were systematically searched in relevant domestic and foreign databas-es and professional association websites.The search time was from the establishment of the database to May 1,2023.Results A total of 14 articles were included,including 5 guidelines,2 clinical practice,2 expert consensus,and 5 systematic reviews.A total of 21 pieces of the best evidence was formed from 5 aspects,including assessment,in-tervention measures,frequency and duration,precautions,and self-management.Conclusion Based on China's na-tional conditions,medical and nursing staff can consider the current situation of patients,and combine with the actu-al clinical situation and the best evidence of ACT in patients with bronchiectasis,and formulate ACT programs for patients with bronchiectasis to improve their clinical outcomes and quality of life.
