摘要
目的 了解老年2型糖尿病住院患者认知衰弱现状并分析其影响因素.为认知衰弱预防与干预提供参考.方法 采用一般资料问卷、日常生活活动能力量表(activities of daily living,ADL)、简易微型营养评估表(mini nutrition assessment-short form,MNA-SF)、老年抑郁评定量表(geriatric depression scale,GDS-5)、匹兹堡睡眠质量指数量表(pittsburgh sleep quality index,PSQI)、简易精神状态量表(mini-mental state examination,MMSE)、中文版Tilburg衰弱量表对上海市第五人民医院294例老年2型糖尿病住院患者进行调查,采用单因素和多因素logis-tic 回归分析认知衰弱的影响因素.结果 共152例(51.7％)老年2型糖尿病住院患者存在认知衰弱;回归分析显示,婚姻状况、有无听力障碍、有无咀嚼功能障碍、有无文娱活动、日常生活活动能力及睡眠质量是认知衰弱的主要影响因素(均P＜0.05).结论 老年2型糖尿病住院患者认知衰弱发生率较高,非在婚、存在听力障碍、咀嚼功能障碍、日常生活活动能力受损、缺乏文娱活动、睡眠质量差者更易发生认知衰弱,医护人员应重视该人群认知衰弱的筛查与干预.
Abstract
Objective To understand the status of cognitive frailty in older inpatients with type 2 diabetes and ana-lyze its influencing factors,so as to provide reference for the prevention and intervention of cognitive frailty.Methods A total of 294 older inpatients with type 2 diabetes from Shanghai Fifth People's Hospital were investigated using the general information questionnaire,Activities of Daily Living(ADL),Mini Nutrition Assessment-Short Form(MNA-SF),Geriatric Depression Scale(GDS-5),Pittsburgh Sleep Quality Index(PSQI),Mini-Mental State Ex-amination(MMSE),and Tilburg Frailty Indicator.The univariate and multivariate logistic regression were adopted to analyze the influencing factors of cognitive frailty.Results A total of 152(51.7％)elderly inpatients with type 2 diabetes had cognitive decline.The results of logistic regression analysis showed that marital status,hearing disor-der,masticatory dysfunction,recreational activities,the activity of daily living,and sleep quality were the main in-fluencing factors of cognitive frailty(P＜0.05).Conclusion The incidence of cognitive frailty in older inpatients with type 2 diabetes is relatively high,and those who are not married,have a hearing disorder,mastication dysfunction,impaired activities of daily living,less recreational activities,and poor sleep quality are more likely to suffer from cognitive frailty.Medical staff should pay more attention to the screening and intervention for their cognitive frailty.
