Research progress on the need for transitional health management in patients with end-stage renal disease
The treatment of patients with end-stage renal disease was characterized by long-term and independ-ence,and out-of-hospital treatment was prone to problems such as infection,heart failure,calcium and phosphorus metabolism disorders,which had higher requirements for disease knowledge,nursing operation ability,and social and family support of patients.Therefore,patients had a higher needs for transitional health management.This arti-cle reviewed the concept,importance of assessment,content and assessment tools of transitional health management needs,in order to provide reference for future interventions related to transitional health management in patients with end-stage renal disease.
