A qualitative study on the promotion and obstacle factors of transition guidance for children with inflammatory bowel disease implemented by medical staff
Objective To understand the promotion and obstacle factors of medical staff in the process of imple-menting transition guidance for children with inflammatory bowel disease(IBD),so as to provide reference for fur-ther implementation of transition guidance in clinical practice.Methods Using the method of purposive sampling,10 nurses and 6 doctors who worked in the department of gastroenterology of 2 tertiary A hospitals in Shanghai from October 2022 to February 2023 were selected for semi-structured interviews,and Colaizzi 7-step analysis method was used to sort out and analyze the data,and describe the theme.Results The promotion and obstacle factors for medi-cal staff to implement transition guidance were extracted.The four sub-themes of the promotion factor theme includ-ed medical staff being aware of the importance of transitioning from children to adults,children's needs for transitio-ning to adults,children's and parents'perceptions of medical care,and information and emotional support for children provided by peer support.The six sub-themes of the obstacle factor theme included improper allocation of human re-sources,single form of transition guidance,imperfect transition guidance content and process,and lack of personal-ized dynamic assessment of children's transition readiness by doctors and nurses,lack of awareness of active health management of children,and excessive involvement of parents in disease management of children.Conclusion There are certain promotion and obstacle factors of medical staff in the process of implementation of transition guidance for children with IBD to adults.Medical staff should face up to the importance of setting up a multidisciplinary team for transition guidance,develop information-based transition guidance tools,improve transition guidance content and work flow,and build a transitional guidance model that is in line with the situation in our country.
