目的 总结外固定支架针位护理最佳证据,为临床护理实践提供参考。方法 基于"6S"证据资源金字塔模型,系统检索 JBI 循证卫生保健中心数据库、BMJ Best Practice、PubMed、Cochrane Library、UpToDate、NICE、医脉通、中国知网、万方知识数据服务平台等数据库中与外固定支架针位护理相关的所有文献,文献类型包括指南、系统评价、专家共识、证据总结、随机对照试验等。检索时限为建库至2022年12月31日。由课题组成员根据相关文献质量评价标准独立进行质量评价,并结合专业判断对最终纳入文献进行提取整合、证据级别评定及汇总。结果 共纳入文献13篇,并汇总13条证据,包括6个方面:外固定支架针位清洗液的选择、护理频次、敷料的选择、针道结痂的管理、是否按摩/按压针道位置及洗浴。结论 该研究总结了外固定支架针位护理的最佳证据,建议临床医务人员结合临床实际选择性应用最佳证据,减少并发症的发生,改善患者结局。
Summary of the best evidence for needle position nursing in orthopedic external fixation stents
Objective To summarize the best evidence for needle position nursing of external fixation brackets,and provide reference for clinical nursing practice.Method Based on the"6S"evidence resource pyramid model,all evidence related to external fixation stent needle care from databases such as JBI Evidence Based Healthcare Center Database,BMJ Best Practice,PubMed,Cochrane Library,UpToDate,NICE,Maimai Tong,CNKI,and Wanfang Knowledge Data Service Platform,including guidelines,system evaluations,expert consensus,evidence summaries,and randomized controlled trials,etc.were systematacially retrieved.The search deadline was from database estab-lishment to December 31,2022.The research group members independently conducted quality evaluation according to the relevant literature quality evaluation standards,and combined professional judgment to extract,integrate,e-valuate the level of evidence,and summarize the final included literature.Result A total of 13 articles were included,and 13 pieces of evidence were summarized,including 6 aspects:Selection of cleaning solution for external fixation stent needle position,nursing frequency,selection of dressings,management of needle scab formation,whether to massage or press the needle position,and bathing.Conclusion This study summarizes the best evidence of external fixation stent needle position nursing,and suggests that clinical healthcare professionals should selectively apply the best evidence according to clinical practice to reduce the occurrence of complications,and improve patient outcomes.
