Relationship between perioperative symptoms and physical activity in patients with lung cancer based on ecological momentary assessment
Objective To explore the change pattern between symptoms changes and physical activity in perioper-ative patients with lung cancerand its influence relationship.Methods A total of 96 lung cancer patients who under-went surgery in the department of thoracic surgery of the Affiliated Hospital of Jiangnan University from April to October 2023were conveniently selected as the researchsubjects by ecological momentary assessment,and the general information questionnaire,symptom ecological moment assessment(EMA)instrument(numerical rating scale for pain,visual analogue scale for fatigue,visual analogue scale for dyspnea,distress thermometer,and stanford sleepi-ness scale)and physical activity(PA)monitoring instrument were used to assess the symptoms(pain,fatigue,psy-chological distress,dyspnea,and somnolence)at 3 time points daily within 1-6 days before and after surgery,and to continuously monitor the physical activity(PA)level(steps,distance,and calories),and statistical analysis was performed.Results Symptoms and physical activity in lung cancer patients showed a negative trend in the periopera-tive period,with differences in the timing of peak symptoms.The results of multilevel linear modeling showed that all 5 symptoms had significant negative influence on PA indicators at the instantaneous level(P<0.001),with the pain effect being the most significant.Perioperative PA levels in women with lung cancer were lower compared with men(P<O.01),and the same level of pain,psychological distress,and drowsiness could resulted in greater decrea-ses in step counts,active distance,and active calories in women.In addition,advanced age was an independent risk factor affecting patients'physical activity levels(P<0.001),and step count,distance traveled,and calories from ac-tivity were more significantly reduced in older patients with pain,but psychological distress had a lesser negative effect on step count and calories from activity in older patients(β=-7.65,-0.236).Conclusion Symptoms and physical activity of lung cancer patients show a dynamic change pattern in the perioperative period,and the more se-rious the symptoms are,the lower thelevel of PA,and there is an interaction effect with patients'age and genderof patients.
