目的 了解医护人员预立医疗照护计划(advance care planning，ACP)推行意愿现状并分析其影响因素。方法 采用分层抽样的方法，于2022年10-11月采用一般资料调查表、医护人员ACP推行意愿调查问卷对四川省成都市某三甲医院肿瘤科、ICU、老年医学科等终末期患者相对多科室的400名医护人员进行调查。采用多元线性回归分析ACP推行意愿的影响因素。结果 回收有效问卷366份，调查对象的ACP推行意愿总均分为(60。86士9。71)分，其中行为意向维度得分为(20。33±3。62)分，处于中等偏上的水平;接触过濒死/死亡患者、接受过死亡教育、家庭对"死亡"话题的讨论频次与氛围是医护人员ACP推行行为意愿的主要影响因素(P＜0。05)。结论 医护人员ACP推行意愿总体上呈现比较积极的态度。ACP的推行受个人及社会多种因素影响，死亡教育是关键因素，同时需要全社会的认可与参与，完善法律法规，才有利于逐步推进ACP的真正实施。
Current situation and influencing factors of medical staffs willingness to implement advance care planning
Objective To understand the current situation of medical staff's willingness to implement advance care planning(ACP)and analyze its influencing factors.Methods Astratifiedsampling method was used to investigate 400 medical staff in the department of oncology,ICU,geriatrics,and other end-stage patients in a tertiary hospital in Chengdu,Sichuan province from October to November 2022 by using the general information questionnaire and the questionnaire of the willingness of medical staff to implement advance care planning.The influencing factors of ACP implementation intention were analyzed by multiple linear regression.Results Three hundred and sixty-six valid ques-tionnaires were collected,andthe total mean score of the respondents'intention to implement ACP was(60.86±9.71)points,of which the behavioural intention dimension score was(20.33±3.62)points,which was in the middle to upper range.Exposure to dying/dying patients,death education,frequency and atmosphere of family discussion on the topic of"death"are the main influencing factors of ACP implementation willingness of medical staff(P＜0.05).Conclusion ACP implementation intention of medical staff generally presents a more positive attitude.The imple-mentation of ACP is affected by many personal and social factors,and death education was the key factor.At the same time,the recognition and participation of the whole society,and improvementof laws and regulations are condu-cive to the real implementation of ACP.
