Latent profile analysis of sleep characteristics and its differences with quality of life in elderly patients with chronic musculoskeletal pain
Objective To explore potential subtypes of sleep quality in elderly patients with chronic musculoskele-tal pain(CMP)based on latent profile analysis(LPA)and to analyze the differences in quality of life among sub-types.Methods A total of 508 elderly patients with CMP who attended the department of painology and department of joint surgery of the First Affiliated Hospital of Xinjiang Medical University from January to July 2023 were select-ed as the research objects by using convenience sampling method, and a questionnaire survey was conducted using the general information questionnaire, the Pittsburgh sleep quality index(PSQI),and the World Health Organization quality of life questionnaire abbreviated version(WHOQOL-BREF)in them. Results The sleep quality of elderly pa-tients with CMP could be classified into 4 subtypes:Low sleep efficiency type(29.70%),good sleep type(51.80%),insufficient sleep time-high medication use type(14.80%),and subjective poor sleep-sleep disorders type(3.70%).The differences in PSQI scores of the patients in the 4 subtypes were statistically significant when compared among the categories(P<0.05).Differences in literacy, degree of existing pain, pain episodes, number of pain sites, oral analgesic medication, and regular exercise status were statistically significant between categories(P<0.05).Differences in WHOQOL-BREF scores among 4 subtypes patients were statistically significant(P<0.05).Conclusion The sleep quality of elderly patients with CMP can be classified into 4 latent categories, and the different groups of categories perform differently in terms of quality of life.Patients with low literacy, severe pain, multiple pain sites, persistent pain episodes,long-term analgesic medication and irregular exercise, and provide rea-sonable sleep hygiene education and psychological support should be focused on,and according to the characteristics of different categories of patients, the reasonable sleep hygiene education and the psychological support are made to better cope with and alleviate the problem of imbalance of sleep homeostasis, and improve the level of quality of life.
