Application effect of intravenous therapy systematic training based on training transfer theory in intravenous therapy nursing specialty alliance

目的 探讨基于培训迁移理论的静脉治疗系统化培训在静疗护理专科联盟的应用效果.方法 通过基线调查、现况分析、文献查阅及小组讨论构建基于培训迁移理论的静脉治疗系统化培训内容,选取广西静脉治疗护理专科联盟医院的71名护士,采用自身前后对照法,比较培训前和培训后1个月、3个月护士的静疗行为规范、培训迁移行为得分及经外周置入中心静脉导管(peripherally inserted central catheter,PICC)并发症发生率.结果 (1)静疗行为规范:规范PICC冲封管及规范PICC拔除2项培训前后比较差异有统计学意义(P＜0.05).(2)PICC并发症发生率:从培训前的8.80％下降至培训后的7.32％,差异有统计学意义(P＜0.05).(3)培训迁移行为:培训前和培训后1个月、3个月评分差异有统计学意义(P＜0.05).结论 基于培训迁移理论的静脉治疗系统化培训能规范护士的静疗行为,提高护士的培训迁移行为,提高并发症处理能力,降低PICC导管并发症的发生,系统化的理论培训较日常零散的培训效果显著,但随着时间推移其应用情况呈下降趋势,提示延续培训的必要性.
Objective To explore the application and effect analysis of systematic intravenous therapy training based on the theory of training transfer in the intravenous nursing specialist alliance.Methods Through baseline in-vestigation,current situation analysis,literature review,and group discussion,a systematic training content of in-travenous therapy based on the theory of training transfer was constructed.A total of 71 nurses from Guangxi Vein Therapy and Nursing Alliance Hospital were selected,and self pre-and post control method was used,and the differences of static therapy behavior,the scores of nurse training transfer behavior,and incidence of peripherally in-serted central catheter(PICC)catheter complications were compared between nurses before training and 1 month and 3 months after training.Results(1)Quiet therapy behavior:There was a statistically significant difference(P＜0.05)in the nurse's flushing and sealing techniques,as well as the PICC puncture point being kept closed for 24 hours before and after training.(2)The incidence of PICC complications:The rate decreased from 8.80％before training to 7.32％after training,with a statistically significant difference(P＜0.05).(3)Nurse training transfer behavior:There were statistically significant differences between the scores before training and 1 month and 3 months after training(P＜0.05).Conclusion The systematic training of intravenous therapy based on the theory of training transfer can standardize nurses'static therapy behavior,improve nurses'training transfer behavior,improve their a-bility to manage complications,and reduce the occurrence of PICC catheter complications.Systematic theoretical training has significant effects compared with scattered daily training,but its application has a downward trend over time,suggesting the necessity of continuous training.
