A mixed study of financial toxicity status and negative experience in patients with colorectal cancer

目的 通过量性和质性相结合的混合研究方法,了解结直肠癌患者经济毒性现状及负性体验.方法 2023年7月-2024年1月,采用便利抽样法选取在浙江省某三甲医院治疗的211例结直肠癌患者为研究对象,使用一般资料调查法、患者报告结局的经济毒性综合评分量表(comprehensive scores for financial toxicity on the pa-tient-reported outcome measures,COST-PROM)进行调查;通过目的抽样法选取12例患者进行半结构式访谈,运用Colaizzi 7步分析法提炼出4个主题.结果 结直肠癌患者经济毒性总分为(26.82±9.33)分.多重线性回归结果显示,家庭月收入、居住地、肿瘤分期、是否化疗是结直肠癌患者经济毒性的影响因素(P＜0.05).质性研究共提炼出4个主题:经济支出多样化、持续性;就业改变加剧经济毒性问题;经济毒性主导的复杂情绪体验;经济资源可及性不足.结论 结直肠癌患者普遍存在经济毒性,且存在较多负性体验.医护人员需加强对经济毒性的主客观评估,通过提供支持性服务和护理,引导患者采取积极的应对方式,扩大自身的社会支持网络,缓解经济毒性水平.
Objective To understand the status of financial toxicity and negative experience of patients with color-ectal cancer by a combination of quantitative and qualitative study.Methods From July 2023 to January 2024,211 pa-tients with colorectal cancer treated in a Class Ⅲ Grade A hospital in Zhejiang province were selected by convenience sampling method,and general data survey method and comprehensive scores for financial toxicity based on the pa-tient-reported outcomes measures(COST-PROM)were used to investigate.A total of 12 patients were selected by the objective sampling method for semi-structured interviews,and 4 themes were extracted by Colaizzi 7-step analy-sis method.Results The total financial toxicity score of colorectal cancer patients was(26.82+9.33)points.The re-sults of multiple linear regression showed that monthly family income,residence,tumor stage,and whether or not chemotherapy were the influencing factors of financial toxicity in patients with colorectal cancer(P＜0.05).The qualitative research had extracted 4 themes:Diversification and sustainability of economic expenditure;Employment changes exacerbate financial toxicity;A complex emotional experience dominated by financial toxicity;Insufficient access to economic resources.Conclusion Financial toxicity is common in patients with colorectal cancer,and there are more negative experiences.Medical staff should strengthen the subjective and objective assessment of financial toxicity,guide patients to adopt positive coping methods by providing supportive services and nursing,expand their own social support network,and alleviate financial toxicity levels.
