摘要
目的 基于主客体互倚模型(actor-partner interdependence model,APIM)分析晚期癌症患者和配偶家庭回避癌症沟通、家庭关怀度对生活质量的影响.方法 于2022年11月-2023年7月,选取在我院肿瘤放疗科住院治疗的晚期癌症患者及其配偶251对作为研究对象,采用一般资料调查表、家庭回避癌症沟通量表(family avoid-ance of communication about cancer scale,FACCS)、家庭关怀度量表及简明生活质量问卷(12-item short form health survey,SF-12)进行调查,建立家庭回避癌症沟通、家庭关怀度对生活质量的APIM.结果 晚期癌症患者与配偶家庭回避癌症沟通、家庭关怀度、生活质量心理维度得分比较,差异有统计学意义(P＜0.05).APIM结果显示,患者家庭回避癌症沟通、家庭关怀度可影响自身及配偶的生活质量(P＜0.01),同时配偶家庭关怀度能够影响双方的生活质量心理维度(P＜0.05).结论 晚期癌症患者及其配偶的生活质量与二者家庭回避癌症沟通、家庭关怀度存在交互影响,提示护理人员应重视晚期癌症患者夫妇疾病沟通以及家庭关怀度的评估,积极探索有效的夫妻沟通技能培训以提高其疾病沟通能力,改善家庭功能,进而提高二者的生活质量.
Abstract
Objective To analyze the effects of family avoidance communication and family caring degree on quali-ty of life in advanced cancer patients and their spouses based on the actor-partner interdependence model(APIM).Methods A total of 251 pairs of advanced cancer patients and their spouses who were hospitalized in the oncology ra-diotherapy department of our hospital were selected as the study objects from November 2022 to July 2023,and were investigated by general data questionnaire,family avoidance of communication about cancer scale(FACCS),family caring scale,and 12-item short form health survey(SF-12),and the APIM of family cancer avoidance communication and family care on quality of life were established.Results There were significant differences in the psychological di-mensions of family avoidance communication,family caring,and quality of life between the patients with advanced cancer and their spouses(P＜0.05).The results of the APIM showed that the degree of family avoidance communica-tion and family care degree could affect the quality of life of themselves and their spouses(P＜0.01),and the degree of family care of the spouses could affect the psychological dimension of their quality of life(P＜0.05).Conclusion The quality of life of patients with advanced cancer and their spouses interacted with family avoidance communication and family caring.It is suggested that nursing staff should attach importance to the evaluation of disease communica-tion and family care degree of couples with advanced cancer patients,and actively explore effective communication skills training for couples to improve the disease communication ability of patients and couples,improve family func-tion,and thus improve the quality of life of both.
