不同健康教育方式在经阴道分娩初产妇盆底肌康复中的效果研究

目的 探讨常规产后指导及健康教育、基于收益框架的健康教育、基于损失框架的健康教育在经阴道分娩初产妇盆底肌康复中的应用效果。方法 选取2022年3月-2023年3月湖南中医药大学第一附属医院收治的120例经阴道分娩初产妇为研究对象,采用计算机随机程序将其分为对照组、收益框架组及损失框架组,各40例,但对照组中途脱落3例,收益框架组中途脱落2例,损失框架组中途脱落2例,最终共纳入113例,其中对照组37例、收益框架组38例,损失框架组38例。对照组给予常规产后指导及健康教育,收益框架组在对照组基础上进行基于收益框架的健康教育,损失框架组在对照组基础上进行基于损失框架的健康教育。干预42 d后,对3组的疾病认知水平、自我效能、依从性进行比较。结果 (1)在疾病认知水平和自我效能方面,干预前,对照组、收益框架组及损失框架组比较,差异均无统计学意义(P>0。05);损失框架组和收益框架组均优于对照组,且损失框架组优于收益框架组,差异均有统计性意义(P<0。05)。(2)在依从性方面,干预前,对照组、收益框架组及损失框架组比较,差异均无统计学意义(P>0。05);损失框架组优于收益框架组及对照组(P<0。05),但收益框架组与对照组比较,差异无统计学意义(P>0。05)。结论 基于信息框架理论的健康教育可提高经阴道分娩初产妇患者的盆底肌康复认知水平、自我效能及锻炼依从性,但未来仍需开展多中心、大样本的临床研究,并对收益框架与损失框架的具体适宜人群进行研究和探讨,以促进基于信息框架理论健康教育的临床推广与应用。
Effect of different health education models on pelvic floor muscle rehabilitation of primiparous women after vaginal delivery
Objective To explore the application effects of routine postpartum guidance and health education,health education based on gain framework,and health education based on loss framework in pelvic floor muscle reha-bilitation of primiparous women with vaginal delivery.Methods A total of 120 primiparous women admitted in the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from March 2022 to March 2023 were selected as the study subjects,and were divided into a control group,a gain-frame group,and a loss-frame group with 40 cases in each group using computer randomization program.However,3 cases dropped off in the control group,2 cases dropped off in the gain frame group,and 2 cases dropped off in the loss frame group.A total of 113 cases were finally enrolled,including 37 cases in the control group,38 cases in the gain-frame group,and 38 cases in the loss-frame group.The control group received routine postpartum guidance and health education,and the gain-frame group received health education based on the gain-frame on the basis of the control group,and the loss-frame group received health education based on the loss-frame on the basis of the control group.After 42 days of interven-tion,the disease cognition level,self-efficacy,and compliance were compared among 3 groups.Results(1)In terms of disease cognition and self-efficacy,there were no statistically significant differences among the control group,the gain-frame group,and the loss-frame group before intervention(P>0.05),and both the loss-frame group and the gain-frame group were better than the control group,with the loss-frame group being superior to the gain-frame group,and the differences were all statistically significant(P<0.05).(2)In terms of compliance,there were no statistically significant differences among the control group,the gain-frame group,and the loss-frame group before intervention(P>0.05),and the loss-frame group was better than the gain-frame group and the control group(P<0.05),but the difference between the gain-frame group and the control group was not statistically significant(P>0.05).Conclusion Health education based on the message framework theory can improve the cognitive level of pelvic floor muscle rehabilitation,self-efficacy,and exercise adherence among primiparous patients with transvaginal deliv-ery.In the future,multi-center and large-sample clinical studies are still needed,as well as studies and research on the specific appropriate populations for the gain frame and loss frame,in order to promote the clinical promotion and application of health education based on the message frame theory.
