聚桂醇泡沫硬化剂注射配合内镜下胶圈套扎术治疗Ⅱ、Ⅲ度内痔的疗效

摘要
目的 分析聚桂醇泡沫硬化剂注射配合内镜下胶圈套扎术治疗Ⅱ、Ⅲ度内痔的临床效果.方法 将我院收治的内痔患者 106 例随机分为两组,每组 53 例.对照组予以内镜下胶圈套扎术治疗,观察组予以聚桂醇泡沫硬化剂注射配合内镜下胶圈套扎术治疗,比较两组手术相关指标、临床疗效、术后肛管压力及术后并发症发生情况.结果 观察组术后痔核脱落时间、术后住院时间明显短于对照组,术后24h VAS评分明显低于对照组(P均<0.05);观察组治疗总有效率明显高于照组(P均<0.05);观察组术后 1 个月肛管静息压、肛管最大收缩压明显低于对照组(P均<0.05);观察组术后并发症发生率明显低于对照组(P均<0.05).结论 聚桂醇泡沫硬化剂注射配合内镜下胶圈套扎术治疗Ⅱ、Ⅲ度内痔,可缩短术后痔核脱落及住院时间,减轻术后疼痛,提高治疗效果,降低肛管压力并能减少术后并发症.
Abstract
Objective To analyze the clinical effect of lauromacrogol sclerosing foam injection combined with endoscopic rubber snare ligation in the treatment of degree Ⅱ and Ⅲ internal hemorrhoids.Methods A total of 106 patients with internal hemorrhoids were randomly divided into the control group and the observation group,with 53 cases in each group.The control group was treated with endoscopic rubber snare ligation,and the observation group was treated with lauromacrogol sclerosing foam injection combined with endoscopic rubber snare ligation.The operation-related indicators,clinical efficacy,postoperative canal pressure and postoperative complications were compared between the two groups.Results The hemorrhoidal nucleus shedding time and hospitalization time in the observation group were significantly shorter than those in the control group,and the VAS score 24 h after surgery was significantly lower than that of the control group(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).The resting pressure of the canal and the maximum systolic blood pressure of the canal in the observation group 1 month after operation were significantly lower than those of the control group.and the incidence rate of postoperative complications in the observation group was significantly lower than that of the control group(P<0.05).Conclusion Lauromacrogol sclerosing foam injection combined with endoscopic rubber snare ligation in the treatment of degree Ⅱ and Ⅲ internal hemorrhoids can shorten the hemorrhoidal nucleus shedding time and hospitalization time,reduce postoperative pain,improve the treatment effect,reduce canal pressure,and reduce postoperative complications.
