Impact of perioperative evaluation and management of elderly healthcare population undergoing surgery
Objective To explore the effect of comprehensive management in the perioperative period of the elderly.Methods Retrospective control study was constructed.The study recruited 906 participants aged over 60 years who underwent non-cardiac surgery at the Second Medical Center of General Hospital of Chinese People's Liberation Army from January 2003 to December 2012 as the control group,and the routine management were administrated;1090 patients who need surgical treatment from January 2013 to December 2022 were recruited as the test group,and the comprehensive management scheme was adopted.The differences of perioperative bleeding,infection,delirium,thrombosis,cardio-cerebrovascular events and patient satisfaction were compared between the two groups.Results The incidence of infection(37.1%vs.42.3%),delirium(28.9%vs.34.5%),thrombosis(23.9%vs.36.5%),cardio-cerebrovascular events(25.6%vs.31.1%),etc.in the patients in the test group was significantly lower than that in the control group after operation(χ2=5.622,7.043,37.548,7.198,P<0.05).The all-cause mortality in the observation group was lower than that in the control group(0.64%vs.1.43%),but the difference was not statistically significant(χ2=3.314,P>0.05).The patient satisfaction of the observation group was significantly higher than that of the control group(100%vs.99.7%),and the difference between the two groups was statistically significant(χ2 = 3.025,P<0.05).Conclusion The application of the comprehensive management program in the elderly perioperative period is effective,which can reduce mortality,infection rate,complications and improve the quality of rehabilitation.
