Observation on the effect of visual trephine articular process arthroplasty under endoscope in the treatment of lumbar disc herniation
Objective To observe the effect of visual trephine articular process arthroplasty under endoscope in the treatment of lumbar disc herniation.Methods A total of 107 patients with lumbar disc herniation who underwent percutaneous foraminal endoscopy treatment in our department from September 2021 to August 2022 were selected.Among them,57 cases were assisted by visual ring saws and 50 cases were assisted by TESSYS technology.The operation time,C-arm fluoroscopy times and visual analogue score(VAS)of pain during articular process shaping were compared between the two groups;VAS score,Oswestry Dysfunction Index(ODI)score before and 3 months after surgery together with the incidence of postoperative complications were also compared between the two groups.Results The number of intraoperative fluoroscopy,VAS score and operation time in the visual circular saw assisted group were lower than those in the TESSYS group(P<0.05).The postoperative VAS and ODI scores in the lower back and legs of the two groups were lower than those before surgery,with statistically significant differences(P<0.05).There was no statistically significant difference in VAS and ODI between the two groups before and after surgery(P>0.05).The incidence of postoperative complications in the visual trephine assisted group was 3.51%(2/57)while 4%(2/50)in the TESSYS group,with no statistically significant difference between the two groups(χ2 =0.017,P=0.894).Conclusion The visual trephine articular process arthroplasty under endoscope has a positive effect on the treatment of lumbar disc herniation.Compared with TESSYS technology,it is easier to operate,can shorten the operation time,reduce radiation exposure and intraoperative pain.
