目的 观察椎间孔镜下可视化环锯关节突成形术辅助治疗腰椎间盘突出症效果。方法 选取 2021 年 9 月~2022 年 8 月于北京市顺义区医院疼痛科接受经皮椎间孔镜治疗的腰椎间盘突出症患者 107 例。其中采用可视化环锯辅助57 例(可视化环锯辅助组),采用经皮椎间孔入路椎间孔镜技术(transforaminal endoscopic spine system,TESSYS)技术 50 例(TESSYS组)。比较两组患者手术时间、术中C臂透视次数、关节突成形时疼痛视觉模拟评分(VAS);比较两组患者术前及术后 3 个月VAS评分、Oswestry功能障碍指数(ODI)评分;比较两组患者术后并发症发生情况。结果 可视化环锯辅助组术中透视次数、关节突成形过程中VAS评分及手术时间均低于TESSYS组[(5。77±0。93)次vs。(13。46±1。58)次、(2。03±0。61)分vs。(2。75±0。74)分、(61。34±13。81)min vs。(68。52±10。96)min],组间差异有统计学意义(t=31。122、5。514、2。950,P<0。05)。手术前,两组患者VAS及ODI组间差异均无统计学意义(P>0。05)。手术后,两组患者下腰背和腿部VAS评分、ODI评分均低于术前,差异有统计学意义(P<0。05);而组间VAS及ODI差异均无统计学意义(P>0。05)。可视化环锯辅助组术后并发症发生率为 3。51%(2/57),TESSYS组并发症发生率为 4%(2/50),两组差异无统计学意义(χ2 = 0。017,P= 0。894)。结论 椎间孔镜下可视化环锯关节突成形术辅助治疗腰椎间盘突出症疗效肯定,与TESSYS技术比较操作更简便,可缩短手术时间,减少放射暴露及术中疼痛。
Observation on the effect of visual trephine articular process arthroplasty under endoscope in the treatment of lumbar disc herniation
Objective To observe the effect of visual trephine articular process arthroplasty under endoscope in the treatment of lumbar disc herniation.Methods A total of 107 patients with lumbar disc herniation who underwent percutaneous foraminal endoscopy treatment in our department from September 2021 to August 2022 were selected.Among them,57 cases were assisted by visual ring saws and 50 cases were assisted by TESSYS technology.The operation time,C-arm fluoroscopy times and visual analogue score(VAS)of pain during articular process shaping were compared between the two groups;VAS score,Oswestry Dysfunction Index(ODI)score before and 3 months after surgery together with the incidence of postoperative complications were also compared between the two groups.Results The number of intraoperative fluoroscopy,VAS score and operation time in the visual circular saw assisted group were lower than those in the TESSYS group(P<0.05).The postoperative VAS and ODI scores in the lower back and legs of the two groups were lower than those before surgery,with statistically significant differences(P<0.05).There was no statistically significant difference in VAS and ODI between the two groups before and after surgery(P>0.05).The incidence of postoperative complications in the visual trephine assisted group was 3.51%(2/57)while 4%(2/50)in the TESSYS group,with no statistically significant difference between the two groups(χ2 =0.017,P=0.894).Conclusion The visual trephine articular process arthroplasty under endoscope has a positive effect on the treatment of lumbar disc herniation.Compared with TESSYS technology,it is easier to operate,can shorten the operation time,reduce radiation exposure and intraoperative pain.
