Relationship between cystatin C,insulin growth factor-1 and left ventricular hypertrophy secondary to type H hypertension
Objective To observe the effects of serum cystatin C(Cys C)and Chemerin on secondary left ventricular hypertrophy(LVH)in patients with primary hypertension.Methods The study subjects were middle-aged and elderly hypertensive patients who visited our hospital from March 2020 to March 2023.According to echocardiography,they were divided into a left ventricular hypertrophy group(LVH group)of 45 cases and non left ventricular hypertrophy group(NLVH group)of 77 cases.During the same period,echocardiography was performed in our hospital,with 60 healthy individuals without hypertension as the control group.Compare general clinical data such as patient course and medication,total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting blood glucose(FPG),urea nitrogen(UN),creatinine(Cr),Cys C,Chemerin,and homocysteine(Hcy).Pearson correlation analysis and logistic regression analysis were used to analyze the influencing factors of left ventricular hypertrophy.Results The BMI,TC,SBP,DBP,Cys C,Chemerin,and Hcy levels in the NLVH and LVH groups were higher than those in the control group.The BMI,SBP,DBP,Cys C,Chemerin,and Hcy levels in the LVH group were higher than those in the NLVH group,and the differences were statistically significant(P<0.05).The number of smokers and Cr levels in the NLVH group were higher than those in the control group,and the differences were statistically significant.However,compared with the LVH group,the differences were not statistically significant.There was no statistically significant difference in age,gender,blood lipids,and other indicators among the three groups.There was no statistically significant difference in the course of disease and the use of antihypertensive drugs between the NLVH group and the LVH group.Cys C and Chemerin are positively correlated with the left ventricular mass index,with correlation coefficients of 0.305 and 0.182,respectively.Multivariate regression analysis showed that SBP,obesity,Cys C,Chemerin,and Hcy are independent risk factors affecting the occurrence of LVH.Conclusion Cys C and Chemerin are involved in the pathological development of left ventricular hypertrophy and are independent risk factors for secondary left ventricular hypertrophy in middle-aged and elderly patients with primary hypertension.
