深部热疗联合恩度、顺铂胸腔热灌注化疗对恶性胸腔积液患者免疫功能、肿瘤标志物影响及短期疗效的临床观察

目的 探讨深部热疗(deep hyperthermia)配合恩度(Endostar)、顺铂胸腔灌注化疗对恶性胸水患者免疫功能、肿瘤标志物的影响及其短期疗效。方法 选取 2020年至 2023年在湖北省第三人民医院肿瘤科治疗的恶性胸水患者 70例,并将其随机分组成观察组与对照组,各 35例。观察组采用深部热疗配合恩度(重组人血管内皮抑制素)、顺铂胸腔热灌注,对照组仅采用单纯恩度、顺铂胸腔热灌注。观察两组患者免疫相关指标(CD4+、CD8+、CD4+/CD8+)、肿瘤标志物(CEA、CA19-9、CA125)、短期疗效及疗程内的毒副反应(ADR)发生率,治疗结束后患者的生活质量改善率。结果 观察组的总体有效率(82。86%)显著高于对照组(48。57%)(P<0。05)。此外,在CD4+和CD4+/CD8+的对照中,观察组均显著大于对照组(P<0。05);另外,两组患者的血清CEA、CA125 和CA19-9 水平都有所下降,而观察组的下降幅度明显大于对照组(P<0。05);观察组的生活质量改善率(77。13%)明显优于对照组(42。85%)(P<0。05),两组不良反应(ADR)发生率无显著性差异(P>0。05)。结论 深部热疗与恩度、顺铂胸腔热灌注化疗相结合,可以提高患者的免疫功能,调节肿瘤标志物水平,对恶性胸水具有良好的短期疗效,能够使患者的生活质量得到有效的提升。
Clinical Observation on Effect of Deep Hyperthermia Combined with Endostar and Cisplatin Intrapleural Hyperthermic Chemotherapy on Immune Function and Tumor Markers and Short-term Curative Effect in Patients with Malignant Pleural Effusion
Objective To investigate the effect of deep hyperthermia combined with Endostar and cisplatin intrapleural infusion chemotherapy on immune function and tumor markers and short-term curative effect in patients with malignant pleural effusion.Methods 70 patients with malignant pleural effusion treated in the oncology department of the Third People's Hospital of Hubei Province from 2020 to 2023 were selected and randomly divided into an observation group and a control group,with 35 patients in each group.The observation group received deep hyperthermia combined with Endostatin(recombinant human Endostatin)and cisplatin intrapleural thermal perfusion,while the control group only received pure Endostatin and cisplatin intrapleural thermal perfusion.The immune-related indexes(CD4+,CD8+,CD4+/CD8+),tumor marker(CEA,CA19-9,CA125),short-term curative effect,the occurrence rate of toxic and side effects during treatment,and the improvement rate of life quality after treatment were observed.Results The total effective rate of the observation group(82.86%)was significantly higher than that of the control group(48.57%)(P<0.05).The observation group was significantly larger than the control group in terms of CD4+and CD4+/CD8+(P<0.05).In addition,the serum levels of CEA,CA125,and CA19-9 in the two groups were decreased,but the decrease of the observation group was significantly greater than that of the control group(P<0.05).The improvement rate of life quality in the observation group(77.13%)was significantly better than that in the control group(42.85%)(P<0.05),and there was no significant difference in the incidence of adverse reactions(ADR)between the two groups(P>0.05).Conclusion Deep hyperthermia combined with Endostar and cisplatin thoracic hyperthermic perfusion chemotherapy can improve the immune function of patients,regulate the level of tumor markers,have a good short-term curative effect on malignant pleural effusion,and effectively improve the life quality of patients.
