目的:探讨 2 型糖尿病(T2DM)合并冠状动脉粥样硬化性心脏病(CAD)患者血清心型脂肪酸结合蛋白(H-FABP)、可溶性ST2(sST2)水平及临床意义.方法:选取确诊为T2DM的患者 89 例为研究对象,根据是否合并CAD将之分为T2DM组(n=49)和T2DM合并CAD组(n=40),并收集同期健康体检者 33 例作为对照组.收集所有受试者性别、年龄、体重指数(BMI)、收缩压(SBP)、舒张压(DBP)等临床资料及总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)等实验室检查指标.分别采用免疫比浊法、酶联免疫吸附试验测定血清H-FABP、sST2 水平.结果:三组受试者性别、年龄、BMI、TC、TG、HDL、LDL、CREA、UA比较,差异无统计学意义(P>0.05);与对照组比较,T2DM组与T2DM合并CAD组SBP、尿素(UREA)及糖化血红蛋白(HbA1c)、N-末端脑利尿钠肽前体(NT-proBNP)及高敏肌钙蛋白T(hs-cTNT)表达水平均明显升高,且T2DM合并CAD组血清NT-proBNP、hs-cTNT表达水平明显高于T2DM组(P<0.05).对照组血清H-FABP、sST2 表达水平与T2DM组比较差异无统计学意义(P>0.05),但T2DM合并CAD组血清H-FABP、sST2 表达水平明显高于对照组与T2DM组(P<0.05).T2DM合并CAD患者血清H-FABP、sST2 水平与NT-proBNP呈正相关,H-FABP水平与sST2 水平也呈正相关.NT-proBNP、hs-cTNT、H-FABP、sST2 诊断T2DM合并CAD的ROC曲线下面积(AUC)分别为 0.75、0.64、0.75、0.71.NT-proBNP、H-FABP 联合检测诊断T2DM合并CAD的敏感性为 78%,特异性为 96%,AUC为 0.87,诊断价值最高.结论:T2DM合并CAD患者血清H-FABP及sST2 表达水平升高,H-FABP与NT-proBNP联合检测在诊断T2DM合并CAD中具有重要价值.
Clinical significances of serum H-FABP and sST2 in patients with type 2 diabetes mellitus complicated with coronary heart disease
Objective To investigate the clinical significances of serum H-FABP and sST2 level in T2DM patients with CAD.Method We enrolled 89 patients with T2DM,according to whether they had CAD or not,the patients were divided into 49 cases in the T2DM group and 40 cases in the T2DM complicated with CAD group,and 33 healthy persons who underwent physical examination in our hospital during the same period were selected as the control group.Clinical data of all subjects including sex,age,BMI,SBP,DBP and the biochemical data such as TC,TG and LDL were collected.The serum H-FABP was detected by latex-enhance immuno-turbidimetric assay and ELISA was used to detect the serum sST2 levels.Results There were no significant differences in sex,age,BMI,TC,TG,LDL and HDL among the three groups(P>0.05).Compared with the control group,T2DM group and T2DM with CAD group had higher SBP,UREA and HbA1c.Serum levels of NT-proBNP,hs-cTNT,H-FABP and sST2 were significantly higher in T2DM with CAD group than those in T2DM group(P<0.05).No significant differences were found in the H-FABP and sST2 among T2DM group and the control group(P>0.05),but serum H-FABP,sST2 levels in T2DM with CAD group were significantly higher than T2DM group and control group(P<0.05).Serum H-FABP,sST2 levels showed significant positive correlated with NT-proBNP level and H-FABP level was positively correlated with sST2 level.The area under the receiver operating characteristic curve(AUC)of NT-proBNP,hs-cTNT,H-FABP and sST2 were 0.75,0.64,0.75 and 0.71,respectively.The combination of NT-proBNP and H-FABP showed the highest AUC of 0.87,the sensitivity and specificity were 78%and 96%,respectively.Conclusion The expression levels of serum H-FABP and sST2 are elevated in patients with T2DM combined with CAD.The combined detection of H-FABP and NT proBNP has important value in the diagnosis of T2DM combined with CAD.
