中文摘要：目的:分析Gordon模式个案管理对老年心脏瓣膜病术后患者抗凝药物用药依从性及自我效能的影响.方法:选取 200 例于南京大学医学院附属鼓楼医院 2022 年 1 月～12 月进行治疗的老年心脏瓣膜病术后患者,随机分为对照组和观察组各 100 例.对照组使用常规护理,观察组在对照组基础上使用Gordon模式个案管理,观察两组患者抗凝药物用药依从性、自我效能、生活质量、抗逆力情况.结果:干预后两组患者抗凝药物用药依从性、自我效能、生活质量、抗逆力等相关指标评分均高于对照组,差异有统计学意义(P＜0.05).结论:对老年心脏瓣膜病术后患者进行Gordon模式个案管理干预,可有效提高患者抗凝药物用药依从性及自我效能.

外文摘要：Objective To analyze the impact of Gordon model case management on the adherence and self-efficacy of anticoagulant medication in elderly patients after heart valve disease surgery.Method 200 elderly patients with postoperative heart valve disease who underwent treatment in our hospital from January 2022 to December 2022 were selected and divided into a control group and an observa-tion group,with 100 cases in each group.The control group received routine care,while the observation group received Gordon mode case management on the basis of the control group.The compliance,self-efficacy,quality of life,and resistance to anticoagulant medi-cation in two groups of patients were observed.Results After intervention,the scores of anticoagulant medication adherence,self-effi-cacy,quality of life,and resistance in both groups of patients were higher than those in the control group(P＜0.05).Conclusion-Gordon model case management intervention for elderly patients with postoperative heart valve disease can effectively improve their ad-herence to anticoagulant medication and self-efficacy.
