Evaluation on the Effect of Health Management of Community Diabetes Patients Under the Mode of Medical Prevention Integration
Objective To explore the application effect of the health management model of medical and preventive integration in type 2 diabetes patients in the community.Methods The registered diabetes patients in two communities in Shanghai were taken as the control group and the intervention group respectively.The control group implemented routine management,while the intervention group implemented integrated medical and preventive health management.The health management outcomes of two groups of patients was compared.Results After 2 years of intervention,the awareness rate of diabetes knowledge in the intervention group was significantly higher than that in the control group(P<0.05).The results of prior event rate ratio adjustment showed that the growth rate of diabetes control behavior in the intervention group was significantly higher than that in the control group(P<0.05).For patients with poor blood glucose control,the intervention group showed significant improvement in blood glucose control after intervention,while the control group did not show significant improvement.Conclusion The health management of medical and prevention integration can more effectively improve the standardized management rate,blood sugar control rate,glycosylated hemoglobin detection rate,glycosylated hemoglobin control rate and related knowledge awareness rate of diabetes patients,so the medical and prevention integration management model is worth promoting in community.
