Application of CT-guided pulmonary puncture combined with metage-nomic sequencing in patients with AIDS and pulmonary infection
Objective To study the application value of CT-guided pulmonary puncture combined with metagenomic sequencing in AIDS patients with lung infection.Methods A total of 60 AIDS patients with pulmonary infection admitted to the Fifth People's Hospital of Ganzhou from November 2021 to May 2023 were selected as research objects.The lung lesion tissues of all subjects were collected by CT-guided pulmonary puncture.Metagenomic sequencing and common culture were performed respectively.The positive detection rate of the two detection methods,the detection rate of bacteria,fungi and tuberculosis of the two detection methods were compared,and the pathogen species detected by the two detection methods.The distribution of pathogenic bacteria in bacterial infection was analyzed.Results The positive detection rate of metagenomic sequencing was 85.00%,and that of common culture was 66.67%,the difference was statistically significant(P<0.05).The detection rate of metagenomic sequencing bacteria was 60.00%,higher than that of common culture(41.67%),the difference was statistically significant(P<0.05).There were no significant differences in the detection rates of Psittacosis,tuberculosis,Marneffei basket bacteria and Cryptococcus between the two detection methods,the differences were not statistically significant(P>0.05).The detection rate of metagenomic sequencing was 46.67%,which was higher than common culture 28.33%,the difference was statistically significant(P<0.05).There were no significant differences in the detection rates of 1 pathogen and 3 pathogen species between the two methods,the differences were not statistically significant(P>0.05).A total of 57 pathogenic bacteria strains were isolated from 40 patients with bacterial infection.In order of proportion from high to low,they were Streptococcus pneumoniae,Klebsiella pneumoniae,Pseudomonas aeruginosa,Escherichia coli and Acinetobacter baumannii,accounting for 29.82%,21.05%,17.54%,14.04%and 12.28%.Conclusion CT-guided pulmonary puncture combined with metagenomic sequencing has high application value in AIDS patients with pulmonary infection,especially in the detection rate of bacteria,and is worthy of popularization and application.In addition,the pathogenic bacteria of AIDS combined with pulmonary bacterial infection are mainly Streptococcus pneu-moniae,Klebsiella pneumoniae,pseudomonas aeruginosa,etc.
