Application of endoscopic surgery through total areola approach in radi-cal thyroidectomy for micro papillary thyroid carcinoma
Objective To evaluate the value of endoscopic surgery through total areola approach in radical thyroidectomy for micro papillary thyroid carcinoma.Methods A total of 80 patients with unilateral micro papillary thyroid carcinoma treated in Xuzhou Central Hospital and the Third Affiliated Hospital of Xuzhou Medical University from September 2019 to June 2023 were retrospectively selected as the study objects.The thyroidectomy methods were isthmus+total thyroidectomy on the affected side and central lymph node dissection.The patients were divided into endoscopic group(40 cases)and open group(40 cases).Endoscopic surgery was performed by the total areola approach in the endoscopic group,and anterior cervical incision was performed in the open group.Surgical indicators,complications and postoperative satisfaction were compared between the two groups.Results The intraoperative bleeding in the endoscopic group was less than that in the open group,the difference was statistically significant(P<0.05).There was no significant difference in drainage volume 24 h after oper-ation between the two groups(P>0.05).The total incision length in the endoscopic group was shorter than that in the open group,and the difference was statistically significant(P<0.05).Pain score 24 h after surgery in endoscopic group was lower than that in open group,the difference was statistically significant(P<0.05).The total incidence of complications in endo-scopic group was lower than that in open group,and the difference was statistically significant(P<0.05).The score of post-operative aesthetic satisfaction in the endoscopic group was higher than that in the open group,and the score of scar self-attention was lower than that in the open group,with statistical significances(P<0.05).Conclusion Endoscopic surgery through total areola approach in radical thyroidectomy for micro papillary thyroid carcinoma has clear operation field,less trauma,less bleeding,ideal postoperative aesthetic effect,and can reduce the complications related to surgery.
