两种全自动血沉仪对红细胞沉降率的测定效果比较

目的 比较普利生LBY-XC-40自动血沉仪(A品牌)和Alifax TEST1全自动快速血沉分析仪(B品牌)测定红细胞沉降率(ESR)的结果,评价不同品牌仪器测定ESR的可靠性。方法 选取2022年1月至2月广州医科大学附属第二医院收集的105份血液标本,分别用两种全自动血沉仪和魏氏法测定ESR,以魏氏法为金标准,比较两种全自动血沉仪和魏氏法的相关性,并进行亚组分析。结果 A、B品牌与魏氏法的测量结果相关性良好(r=0。890、0。902,P<0。05),与金标准的一致性较好。A品牌与魏氏法的测量结果比较,差异无统计学意义(P>0。05),而B品牌与魏氏法的测量结果比较,差异有统计学意义(P<0。05)。进一步对B品牌进行亚组分析,当ESR<20mm/h时,B品牌测量结果高于魏氏法,差异有统计学意义(P<0。05);当ESR≥20mm/h时,B品牌测量结果与魏氏法比较,差异无统计学意义(P>0。05);当血红蛋白≤120g/L时,B品牌的检测结果低于魏氏法,差异有统计学意义(P<0。05),血红蛋白>120g/L时,B品牌与魏氏法比较,差异无统计学意义(P>0。05)。结论 A品牌与魏氏法测定ESR的结果相似,临床上可用于代替魏氏法快速测定ESR;B品牌与魏氏法测定ESR的结果相关性较好,但需重新建立相匹配的参考区间才更适用于临床测定ESR。
Comparison of the effect of two kinds of automatic sedimentation meter on erythrocyte sedimentation rate
Objective To compare results of the determination of erythrocyte sedimentation rate(ESR)by Precil LBY-XC-40 automatic sedimentation rate analyzer(Brand A)and Alifax TEST1 automatic rapid sedimentation rate analyzer(Brand B),and evaluate the reliability of the measurement of ESR by different brands.Methods A total of 105 blood samples collected from the Second Affiliated Hospital of Guangzhou Medical University from January to February 2022 were selected to determine ESR by two kinds of automatic sedimentation system and Westergren method.The Westergren method as the gold standard,the correlation between the two kinds of automatic sedimentation meter and the Westergren method was compared,and the subgroup analysis was carried out.Results Brand A and B had good correlation with the measurement results of the Westergren method(r=0.890,0.902,P<0.05),and were in good agreement with the gold standard.There was no significant difference between the measurement results of Brand A and the Westergren method(P>0.05),while there was statistically significant difference between the measurement results of Brand B and the Wester-gren method(P<0.05).Further subgroup analysis of Brand B showed that when ESR<20 mm/h,the measurement result of Brand B was higher than that of Westergren method,and the difference was statistically significant(P<0.05).When ESR≥20 mm/h,there was no significant difference between the measurement results of Brand B and that of Westergren method(P>0.05).When hemoglobin ≤120 g/L,the detection result of Brand B was lower than that of Westergren method,the dif-ference was statistically significant(P<0.05);when hemoglobin>120 g/L,there was no statistically significant difference between Brand B and Westergren method(P>0.05).Conclusion The results of Brand A are similar to those of the Wester-gren method for ESR measurement,and can be used to replace the Westergren method for rapid ESR measurement in clinical practice.Brand B has a good correlation with the results of Westergren method for ESR measurement,but it needs to re-establish a matching reference interval to be more suitable for clinical ESR measurement.
