Objective To investigate the diagnostic value of endometrial thickness in early prediction of endometrial disease in postmenopausal women. Methods A total of 313 postmenopausal women who underwent hysteroscopy in Binzhou Medical University Hospital from January 2020 to December 2021 were retrospectively included in the study.Clinical data of the patients were collected and divided into three groups according to whether the endometrial thickness of the patients was ≥ 5 mm and whether there was abnormal postmenopausal uterine bleeding measured by vaginal ultrasound.Group A consisted of 102 patients with abnormal uterine bleeding and endometrial thickness ≥5 mm,group B consisted of 53 patients with abnormal uterine bleeding but endometrial thickness<5 mm,and group C consisted of 158 patients with no abnormal uterine bleeding but endometrial thickness≥ 5 mm.The clinical data of the three groups were compared, and subgroup analysis was conducted for patients in group C according to whether intrauterine lesions occurred, and the differences in clinical data were observed.ROC curve was used to analyze the diagnostic value of endometrial thickness in early prediction of intrauterine lesions in postmenopausal women. Results Among the three groups, endometrial polyps accounted for the most(48.56%),among which 63.29%were found in group C.There were statistically significant differences in endometrial thickness and endometrial pathology among the three groups(P<0.05).There were no statistically significant differences in endometrial thickness between group A and group C(P>0.05),but there were statistically significant differences in endometrial pathology between the three groups(P<0.05).Subgroup analysis of group C showed that the endometrial thickness of patients with endometrial lesions was greater than that of those without endometrial lesions, and the difference was statistically significant(P<0.05).ROC curve analysis showed that the AUC of postmenopausal endometrial thickness in early prediction of intrauterine lesions was 0.801,the sensitivity was 58.87%,the specificity was 94.12%,and the cutoff value was 0.85 cm.Conclusion For postmenopausal women without abnormal uterine bleeding, vaginal ultrasonography of endometrial thickness can achieve a good early warning of endometrial lesions, which can be used to avoid unnecessary invasive operations and improve the diagnostic efficiency.
