Analysis of unreasonable medical orders in the pharmacy intravenous ad-mixture services of the First Affiliated Hospital of Anhui University of Chinese Medicine in 2022
Objective To analyze the reasons and intervention status of unreasonable medication orders from the pharmacy intravenous admixture services(PIVAS)in the the First Affiliated Hospital of Anhui University of Chinese Medicine in 2022,provide reference for pharmacists to review prescriptions reasonably and promote clinical rational medication.Methods The intervention records of unreasonable drugs prescribed by hospitalized patients in PIVAS 2022 were collected,the review and intervention of unreasonable medical orders,ward distribution,type of medical orders were analyzed,the causes of unrea-sonable medical orders,and put forward feasible countermeasures were summarized.Results A total of 172 unreasonable medication orders were found,with an unqualified rate of 0.024%.Unreasonable medical advice successfully intervened in 168 groups,with a success rate of 97.67%.The top ranked areas for unreasonable medical orders were Brain Disease Zone 1,Brain Disease Zone 2,Cardiovascular Medicine Zone 2,Rheumatology Medicine Zone 2,and other areas,accounting for a total of 79.65%,making them the main areas for unreasonable drug use.The types with a relatively high proportion of unrea-sonable medical orders were inappropriate solvent volume and inappropriate solvent selection,accounting for 73.26%of the total number of unreasonable medical orders,which were the main types.Conclusion PIVAS pharmacists should enhance the awareness of rational drug use,timely find and correct unreasonable medical orders,and assist clinicians in reasonable drug treatment.
