Objective To investigate the clinical effect of Angio-OCT-guided IQ577TMIRIDEX micropulse laser combined with anti-angiogenesis drugs in the treatment of diabetic macular edema. Methods The clinical data of 60 patients with dia-betic macular edema admitted to the Department of Ophthalmology,People's Hospital of Leping from January 2019 to Jan-uary 2021 were retrospectively analyzed.According to different treatment regimens, they were divided into combined treat-ment group(30 cases)and simple anti-vascular endothelial growth factor(VEGF)group(30 cases). The patients in the com-bined treatment group were treated with Angio-OCT-guided IQ577TMIRIDEX micropulse laser combined with anti-VEGF drugs, and the patients in the simple anti-VEGF group were treated with intravitreal anti-VEGF drugs. The best corrected visual acuity(BCVA),central macular thickness(CMT),foveal avascular zone(FAZ),mean vessel density(VD)of superficial capillary plexus(SCP)and deep capillary plexus(DCP)were compared between the two groups, and the safety of treatment was evaluated. Results The BCVA at 1,2 and 3 months of treatment in the two groups were higher than those before treat-ment,the BCVA at 2 and 3 months of treatment in the two groups were higher than those at 1 month of treatment, and the BCVA at 3 months of treatment in the two groups were higher than those at 2 months of treatment, and the differ-ences were statistically significant(P<0.05).After 2 and 3 months of treatment, the BCVA of the simple anti-VEGF group were lower than those of the combined treatment group at the same time, and the differences were statistically significant(P<0.05).The CMT at 1,2 and 3 months of treatment in the two groups were lower than those before treatment, the CMT of the two groups at 2 and 3 months of treatment in the two groups were lower than those at 1 month of treatment, and the CMT at 3 months of treatment in the two groups were lower than those at 2 months of treatment, and the differences were statisti-cally significant(P<0.05).At 1,2 and 3 months after treatment, the CMT of the simple anti-VEGF group were higher than those of the combined treatment group at the same time point, and the difference were statistically significant(P<0.05).There were no statistically significant differences in FAZ,SCP VD and DCP VD between the two groups before and after treatment(P>0.05).There were no obvious ocular or systemic adverse reactions in the two groups. Conclusion In the treat-ment of diabetic macular edema, the combination of AngO-OCT-guided IQ577TMIRIDEX micropulse laser and anti-VEGF drugs can effectively improve the visual acuity of patients, reduce the thickness of the macular fovea, and improve the thera-peutic effect of patients. The method is simple and safe, which is worthy of clinical application and promotion.
