Observation on the effect of individualized comprehensive urinary control management for neurogenic bladder after spinal cord injury based on urodynamics and bladder safety capacity
Objective To explore the application effect of individualized comprehensive urinary control management based on urodynamics and bladder safety capacity in patients with neurogenic bladder after spinal cord injury.Methods A total of sixty patients with neurogenic bladder after spinal cord injury admitted to Guangdong Provincial Work Injury Rehabilitation Hospital from July 2021 to July 2023 were selected as the study subjects.They were divided into observation group and con-trol group using random number table method,with 30 cases in each group.The control group adopted urine control man-agement based on urodynamics,while the observation group adopted individualized comprehensive urine control manage-ment guided by bladder volume scanners on the basis of the control group.Both groups continued to intervene and observe for 4 weeks.Urodynamic indicators,changes in neurogenic bladder symptoms,incidence of adverse events,urinary tract in-fections,and quality of life between two groups of patients were compared.Results The results showed that after interven-tion,the maximum urine flow rate,maximum urination volume,and detrusor pressure during filling period in the observation group were higher than those in the control group,while bladder pressure was lower than that in the control group,and the differences were statistically significant(P<0.05).After intervention,the neurological bladder symptom scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse events and urinary tract infections in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The observation group's health survey summary table(the MOS item short from health survey,SF-36)after intervention showed higher scores than the control group,and the differences were statistically significant(P<0.05).Conclusion A personalized urine control management plan based on urodynamics and bladder safety capacity can improve urodynamics and neurogenic bladder symptoms in pa-tients with neurogenic bladder after spinal cord injury,reduce the incidence of adverse events and urinary tract infections,and promote the improvement of patient quality of life.
