摘要
目的 探讨身痛逐瘀汤加减方对腰椎间盘突出症(LDH)患者炎症因子水平及疼痛的影响.方法 选择2022年3月至2023年7月于南昌市洪都中医院治疗的93例LDH患者为研究对象,采用随机数字表法分为试验组(n=47)与对照组(n=46).对照组予塞来昔布治疗,于此基础上试验组予身痛逐瘀汤加减方治疗,持续干预3周.比较两组患者的疼痛程度、腰椎功能、炎症因子水平、氧化应激反应与生活质量.结果 治疗后,试验组的视觉模拟评分法(VAS)评分与Oswestry功能障碍指数(ODI)低于对照组,差异有统计学意义(P＜0.05).试验组治疗后的白细胞介素-1 β、肿瘤坏死因子-a、丙二醛和白细胞介素-6均低于对照组,SOD高于对照组,差异有统计学意义(P＜0.05).试验组治疗后的健康调查简表(SF-36)评分高于对照组,差异有统计学意义(P＜0.05).结论 身痛逐瘀汤加减方治疗LDH可降低炎症与氧化应激反应,减轻疼痛程度,改善腰椎功能及生活质量.
Abstract
Objective To explore the effect of modified Shentong Zhuyu Decoction on the levels of inflammatory factors and pain in patients with lumbar disc herniation(LDH).Methods A total of 93 patients with LDH treated in Nanchang Hongdu Traditional Chinese Medicine Hospital in Nanchang from March 2022 to July 2023 were selected as the research subjects and randomly divided into test group(n=47)and control group(n=46)using a random number table method.The control group was treated with Celecoxib,and on this basis,the test group was treated with modified Shentong Zhuyu Decoction for 3 weeks.The pain severity,lumbar function,inflammatory factor levels,oxidative stress response,and quality of life were compared between the two groups.Results After treatment,the visual analog scale(VAS)score and Oswestry disability in-dex(ODI)score in the test group were lower than those in the control group,and the differences were statistically significant(P＜0.05).The levels of interleukin-1 β,tumor necrosis factor-α,malondialdehyde,and interleukin-6 in the test group were lower than those in the control group,while the level of superoxide dismutase(SOD)was higher than that in the control group,and the differences were statistically significant(P＜0.05).The MOS 36-item short from health survey(SF-36)score in the test group after treatment was higher than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion The modified Shentong Zhuyu Decoction can reduce inflammation and oxidative stress reactions,allevi-ate pain severity,improve lumbar function and quality of life in the treatment of LDH.
