Effect of Dexmedetomidine combined with different anesthesia depths on cognitive function and hemodynamics in elderly patients undergoing total hip arthroplasty
Objective To investigate the effects of Dexmedetomidine combined with different anesthesia depths on cognitive function and hemodynamics in elderly patients undergoing total hip arthroplasty(THA).Methods A retrospective analysis was conducted on the data of 80 elderly THA patients admitted to Yingtan People's Hospital from June 2021 to June 2023.They were divided into group A(BIS value 40-49,n=26),group B(BIS value 50-59,n=27),and group C(BIS value 60-69,n=27)based on the bispectral index(BIS).Three groups of patients with different anesthesia depths were treated with THA,and Dexmedetomidine combined anesthesia was used during the surgery.The cognitive function,hemodynamics,and safety among three groups.Results There were no statistically significant differences in preoperative cognitive function scores,hemodynamics,and incidence of adverse reactions among the three groups(P>0.05).The cognitive function scores of the three groups at 12 h after surgery were lower than those before surgery,and cognitive function scores of the three groups at 24,48,72 h after surgery were higher than those at 12 h after surgery,the cognitive function scores of group B at all time points after surgery were higher than those of group A and C,and the differences were statistically significant(P<0.05).The systolic blood pressure and diastolic blood pressure of group A and C at different time points after surgery were lower than those before surgery,and the heart rate of group A and C at different time points after surgery were higher than those before surgery,and the systolic blood pressure and diastolic blood pressure of group A and C at different time points after surgery were lower than those of group B,and the heart rate of group A and C at different time points after surgery were higher than those of group B,with statistical significances(P<0.05).Conclusion The use of Dexmedetomidine combined with a BIS value of 50-59 for anesthesia depth in elderly THA patients has the best effect,which is beneficial for maintaining hemodynamic stability,improving cognitive function,and not increasing adverse reactions.It is worthy of widespread clinical application.
