Application effect of Arantius catheter-guided cephalic approach in la-paroscopic left hemihepatectomy

摘要
目的 探讨经Arantius导管引导头侧入路腹腔镜左半肝切除术在临床中的应用价值.方法 选取2020年1月至2023年12月宜春市人民医院收治的40例腹腔镜左半肝切除术患者作为研究对象,按照随机数字表法分为观察组(20例)与对照组(20例).观察组采用经Arantius导管引导头侧入路腹腔镜左半肝切除术,对照组采用经尾侧入路腹腔镜左半肝切除术.比较两组患者的手术时间、术中出血量、中转开放率、住院时间、术后并发症总发生率及术后1、3、7 d总胆红素(TBil)和丙氨酸转氨酶(ALT)变化情况.结果 观察组术中出血量低于对照组,手术时间、住院时间短于对照组,术后1、3 d TBil水平,术后1、3、7 d ALT水平低于对照组,差异有统计学意义(P＜0.05);两组患者中转开放率、术后并发症总发生率比较,差异无统计学意义(P＞0.05).结论 经Arantius导管引导头侧入路腹腔镜左半肝切除术是一种创伤小、恢复快,而且安全有效的方法,可作为左肝叶疾病手术方式的有效选择.
Abstract
Objective To investigate the clinical value of Arantius catheters-guided cephalic approach laparoscopic left hemihepatectomy.Methods A total of 40 patients with laparoscopic left hemihepatectomy treated in Yichun People's Hospi-tal from January 2020 to December 2023 were selected as the research objects,and they were divided into observation group(20 cases)and control group(20 cases)according to random number table method.The observation group underwent laparo-scopic left hemihepatectomy guided by Arantius catheter.The control group underwent laparoscopic left hemihepatectomy via caudal approach.The operative time,intraoperative blood loss,conversion opening rate,length of hospital stay,total in-cidence of postoperative complications,and the changes of total bilirubin(TBil)and alanine transaminase(ALT)at 1,3 and 7 days after surgery were compared between the two groups.Results The intraoperative blood loss in the observation group was lower than that in the control group,the operation time and hospital stay were shorter than those in the control group,and the level of TBil at 1 and 3 days after surgery and ALT at 1,3 and 7 days after surgery were lower than those in the con-trol group,the differences were statistically significant(P＜0.05).There were no significant differences in the conversion opening rate and the total incidence of postoperative complications between the two groups(P＞0.05).Conclusion The Arantius catheter-guided cephalic approach of laparoscopic left hemihepatectomy is a safe and effective method with less trauma,rapid recovery,and can be used as an effective surgical option for left hepatic diseases.
