摘要

目的 探讨血清肾损伤分子1(KIM-1)、胱抑素-C(Cys-C)、尿微量清蛋白(UmAlb)检测对早期糖尿病肾病(DN)的诊断价值.方法 选取2019年5月至2022年4月九江市中医医院收治的114例糖尿病患者作为研究对象,评估患者早期DN发生情况并分为早期DN发生组、未发生组;检测并比较两组患者血清KIM-1、Cys-C、U-mAlh水平,分析血清KIM-1、Cys-C、UmAlb联合检测对早期DN的诊断价值.结果 114例糖尿病患者中有37例发生早期DN,发生率为32.46%;早期DN发生组血清KIM-1、Cys-C、UmAlb水平均高于未发生组,差异有统计学意义(P<0.05);绘制ROC曲线显示,血清KIM-1、Cys-C、UmAlb单独及联合检测诊断早期DN的AUC>0.080,均有一定诊断价值,且联合检测诊断价值最高.结论 联合检测血清KIM-1、Cys-C、UmAlb诊断早期DN有较高应用价值,对临床诊治有重要指导意义.

Abstract

Objective To investigate the diagnostic value of serum kidney injury molecule 1(KIM-1),cystatin C(Cys-C)and urinary microalbumin(UmAlb)in early diabetic nephropathy(DN).Methods A total of 114 diabetic patients admitted to Jiujiang Hospital of Traditional Chinese Medicine from May 2019 to April 2022 were selected as the study objects to evalu-ate the occurrence of early DN and divided into early DN occurrence group and non-occurrence group.The levels of serum KIM-1,Cys-C and UmAlb were detected and compared between the two groups,and the value of combined detection of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN were analyzed.Results Early DN occurred in 37 of 114 dia-betic patients(32.46%).The levels of KIM-1,Cys-C and UmAlb in the early DN group were higher than those of the non-DN group,and the differences were statistically significant(P<0.05).The ROC curve showed that the AUC value of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN were more than 0.080,which had certain diagnostic value,and the combined detection had the highest diagnostic value.Conclusion The combined detection of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN has high application value,and has important guiding significance for clinical diag-nosis and treatment.
