摘要
银屑病是一种常见的慢性、复发性、炎症性、系统性疾病,主要表现在皮肤和关节,临床上治疗银屑病的方法主要包括传统药物、光疗和生物制剂等.然而,传统药物缺乏选择性和特异性,用于银屑病治疗往往不能达到预期的治疗效果,生物制剂作为靶向治疗药物使银屑病的治疗进入了新的时代,随着生物制剂的出现,大部分患者取得了良好的疗效,但仍有部分患者未见明显效果.近年来,越来越多的专家学者提出联合治疗方案,相比单一药物治疗,生物制剂联合传统药物用于银屑病患者可以提高治疗效果.肿瘤坏死因子α(TNF-α)拮抗剂联合传统药物治疗斑块型银屑病、关节病型银屑病、脓疱型银屑病取得了较好的疗效;白细胞介素(IL)-17A拮抗剂联合传统药物治疗脓疱型银屑病、红皮病型银屑病、斑块型银屑病等也逐渐被报道;IL-23拮抗剂联合传统药物治疗关节病型银屑病、掌跖脓疱病等也逐渐被证实.本文对近年来生物制剂联合传统药物治疗银屑病的有效性及安全性进行综述.
Abstract
Psoriasis is a common chronic,recurrent,inflammatory,and systemic disease,mainly manifested in the skin and joints.The main methods for treating psoriasis in clinical practice include traditional drugs,phototherapy,and biological a-gents.However,traditional drugs lack selectivity and specificity,and their use in the treatment of psoriasis often fails to achieve the expected therapeutic effects.As targeted therapeutic drugs,biological agents have entered a new era in the treat-ment of psoriasis.With the emergence of biological agents,most patients achieved good therapeutic effects,but there are still some patients who did not achieve significant results.In recent years,more and more experts and scholars have proposed a combination therapy plan.Compared to single drug therapy,the combination of biological agents and traditional drugs can improve the treatment effect of psoriasis patients.Tumor necrosis factor-α antagonists combined with traditional drugs,has achieved good efficacy in the treatment of plaque psoriasis,joint psoriasis,and pustular psoriasis.The combination of inter-leukin(IL)-17A antagonists and traditional drugs in the treatment of pustular psoriasis,erythrodermic psoriasis,and plaque psoriasis has gradually been reported.The combination of interleukin-23 antagonists and traditional drugs has gradually been proven to treat joint psoriasis,palmoplantar pustulosis,and other diseases.This article reviews the effectiveness and safety of biological agents combined with traditional drugs in the treatment of psoriasis in recent years.
