目的 研究电针联合认知功能训练在重型颅脑外伤患者中的应用及对认知水平的影响.方法 选取2022年11月至2023年8月南方医院赣州医院(赣州市人民医院)收治的50例重型颅脑外伤患者为研究对象,采用抽签法将患者分为观察组(n=25)和对照组(n=25).对照组患者采用常规基础治疗与认知功能训练,观察组在对照组方法的基础上联合电针治疗.比较两组患者的临床疗效、简易智能精神状态检查(MMSE)、蒙特利尔认知评估(MoCA)、韦氏成人智力量表数字广度测验(WAIS-DST)、连线测试(TMT)、改良Barthel指数(MBI),以及随访6个月的不良反应发生情况与死亡率.结果 观察组的总有效率(88.00%)高于对照组(64.00%),差异有统计学意义(P<0.05).两组患者治疗后的MMSE与MoCA评分均高于本组治疗前,且观察组患者治疗后的MMSE与MoCA评分均高于对照组,差异有统计学意义(P<0.05).两组患者治疗后的WAIS-DST和MBI评分均高于本组治疗前,TMT时间短于本组治疗前,且观察组治疗后的WAIS-DST和MBI评分均高于对照组,TMT时间短于对照组,差异有统计学意义(P<0.05).随访6个月,两组患者的不良反应总发生率与死亡率比较,差异无统计学意义(P>0.05).结论 电针联合认知功能训练在重型颅脑外伤患者中的应用能有效改善患者认知、注意力、短时记忆及日常生活能力水平,具有一定的安全性,为患者的康复治疗提供了新思路.
Objective To study the application of electroacupuncture combined with cognitive function training in patients with severe craniocerebral trauma and its effect on cognitive level.Methods A total of 50 patients with severe craniocerebral trauma admitted to Nanfang Hospital Ganzhou Hospital(Ganzhou People's Hospital)from November 2022 to August 2023 were selected as the research objects.And they were divided into observation group(n=25)and control group(n=25)by drawing lots.The control group was treated with conventional basic treatment and cognitive function training,and the obser-vation group was treated with electroacupuncture on the basis of the control group.The clinical efficacy,mini-mental state examination(MMSE),Montreal cognitive assessment(MoCA),Wechsler adult intelligence scale digit span test(WAIS-DST),trail making test(TMT),modified Barthel index(MBI),and adverse reactions and mortality after 6 months of follow-up were compared between the two groups.Results The total effective rate of the observation group(88.00%)was higher than that of the control group(64.00%),and the difference was statistically significant(P<0.05).The MMSE and MoCA scores of the two groups after treatment were higher than those before treatment,and the MMSE and MoCA scores of the observation group after treatment were higher than those of the control group,and the differences were statistically significant(P<0.05).The WAIS-DST and MBI scores of the two groups were higher than those before treatment,the TMT time in both groups after treatment were shorter than those before treatment,and the WAIS-DST and MBI scores of the observation group after treat-ment were higher than those of the control group,the TMT time was shorter than that of the control group,and the differences were statistically significant(P<0.05).After 6 months of follow-up,there were no significant differences in the total inci-dence of adverse reactions and mortality between the two groups(P>0.05).Conclusion The application of electroacupunc-ture combined with cognitive function training in patients with severe craniocerebral trauma can effectively improve the level of cognition,attention,short-term memory and daily living ability of patients,with a certain safety,and provides a new idea for the rehabilitation treatment of patients.
