Objective To analyze the relationship between seminal parameters and intrauterine insemination(IUI)pregnan-cy outcome,and to discuss whether the influencing factors could predict IUI pregnancy outcome in order to provide clinical data support.Methods A retrospective analysis was performed on 500 couples in Nanping People's Hospital from January 2020 to November 2023,with a total of 812 treatment cycles.The 812 treatment cycles were divided into pregnant group(n=140)and non-pregnant group(n=672)according to whether clinical pregnancy was present.Clinical data of the two groups were collected and compared.It included the age of the woman and the man,infertility years,semen parameters of the man(abstinence time,semen volume before treatment,sperm concentration before treatment,forward motile sperm activity rate before treatment,forward motile sperm concentration before treatment,sperm concentration after treatment,forward motile sperm activity rate after treatment,forward motile sperm concentration after treatment,normal sperm form rate,sperm recovery rate and semen fluid level of transformation).Single factor analysis of clinical data was used.Matchit package in R language was used to match the statistically significant data in the single factor analysis.Multiple logistic regression model was used to analyze the factors affecting the clinical pregnancy success rate.The receiver operatorcharacteristic curve of subjects was drawn to analyze the efficiency of favorable factors in predicting pregnancy outcomes.Results Univariate anal-ysis showed that the age of male and female in the pregnant group were lower than those in the non-pregnant group,the differences were statistically significant(P<0.05).There were no significant differences in infertility years and male se-men parameters between the two groups(P>0.05).The results of the R language MatchIt package male age 1∶3 preference matching analysis showed no statistical significance(P>0.05),and the matched samples were more balanced.Multivariate logistic regression analysis of semen parameters showed that the activity rate of forward motile sperm before treatment and the concentration of forward motile sperm after treatment were influential factors for successful pregnancy.The optimal threshold for predicting the success rate of clinical pregnancy was 33.50%,and the sensitivity and specificity were 81.0%,67.1%,and area under curve was 0.606.After treatment,the optimal threshold of forward motile sperm concentration in pre-dicting the success rate of clinical pregnancy was 38 × 10 6/ml,the sensitivity was 78.4%,the specificity was 67.5%,and the area under the curve was 0.609.The sensitivity,specificity and area under the curve were 81.5%,68.8%and 0.623 respec-tively in predicting the success rate of clinical pregnancy.Conclusion The age of both spouses has adverse effects on the pregnancy rate.The forward motility rate of sperm during insemination and the concentration of forward motility sperm after treatment are important factors affecting the clinical pregnancy rate of IUI couples.The combined prediction of the two has certain clinical value in predicting the pregnancy outcome of IUI couples.
