湘潭市社区2型糖尿病患者达标现状及影响因素

摘要
目的:了解湘潭市社区卫生服务机构2型糖尿病患者血糖、血脂及血压控制现状,分析其影响因素,为基层糖尿病患者心血管病危险因素综合管理提供参考.方法:选取2021年10月至2022年10月湘潭市社区卫生服务中心登记管理的2型糖尿病患者889例作为研究对象,根据《国家基层糖尿病防治管理指南(2022)》标准评价血糖、血压、血脂达标情况,采用卡方检验、二分类Logistic回归分析相关影响因素.结果:湘潭市2型糖尿病患者糖化血红蛋白达标率为36.33%,血压达标率为38.02%,血脂达标率为22.61%,血糖血压血脂三者均达标的患者仅占 4.00%,合并高血压和(或)超重/肥胖者血糖、血压、血脂均不达标的风险增加(P＜0.05).结论:湘潭市社区卫生服务机构2型糖尿病患者血糖、血压、血脂达标率较低,需要积极探索改善社区2型糖尿病管理状况的新模式.
Abstract
Objective:To investigate the blood glucose,blood lipid and blood pressure of patients with type 2 diabetes in community health service centers in Xiangtan and the related influencing factors,to provide reference for comprehensive management of cardiovascular risk factors in community diabetic patients.Methods:We recruited 889 type 2 diabetes patients who were registered and managed in community health service centers in Xiangtan from October 2021 to October 2022 as the subjects.The blood glucose,blood pressure and blood lipid control rate according to the National Guidelines for the Prevention and Management of Diabetes(2022).Chi-square test and binary Logistic regression were used for statistical analysis.Results:The HbA1c control rate is 36.33%,blood pressure control rate is 38.02%,blood lipid control rate is 22.61%,and only 4.00%of patients met the standard of blood glucose,blood pressure and blood lipid,and the risk of patients with hypertension or overweight/obesity was higher(P＜0.05).Conclusion:The control rate of blood glucose,blood pressure and blood lipid of patients with type 2 diabetes in community health service centers in Xiangtan is low,it is necessary to actively explore a new model to improve the management of type 2 diabetes in the community.
