成人幽门螺旋杆菌感染与空腹血糖、血脂、肝功能相关指标的关系研究

摘要
目的:通过检测体检人群幽门螺旋杆菌(Hp)的感染情况,分析成人Hp感染与空腹血糖(FPG)、血脂及肝功能相关指标的关系.方法:连续入选自2021年1月至2023年1月在安徽医科大学附属滁州医院体检并行幽门螺旋杆菌检查的成人共2 972例,根据其Hp感染情况分为Hp阴性组、Ⅰ型Hp组和Ⅱ型Hp组.同步检测其FPG、血脂及肝功能相关指标并分析其数值的组间差异,使用无序多分类Logistic回归分析Hp感染与FPG、血脂及肝功能相关指标的关系.结果:方差分析结果显示,Ⅰ型Hp组与Ⅱ型Hp组的FPG、低密度脂蛋白胆固醇(LDL-C)水平均显著高于Hp阴性组,P均<0.05.此外,与Hp阴性组相比,Ⅰ型Hp组白蛋白(ALB)水平则显著增高,P<0.001;Ⅱ型Hp组间接胆红素(IBIL)水平亦显著增高,P<0.05;而Ⅱ型Hp组高密度脂蛋白胆固醇(HDL-C)水平显著降低,P<0.05.无序多分类Logistic回归分析结果显示,以Hp阴性组为参照,FPG与LDL-C是患Ⅰ型Hp的危险因素,其值越高,患Ⅰ型Hp的可能性越大,OR值分别为1.253(95%CI 1.163~1.351)和1.204(95%CI 1.071~1.354);而ALB水平升高是患Ⅰ型Hp的保护因素,OR值为0.933(95%CI 0.903~0.963).同时,以Hp阴性组为参照,FPG与LDL-C亦是患Ⅱ型Hp的危险因素,OR值分别为1.205(95%CI 1.091~1.330)和1.235(95%CI 1.049~1.453).结论:FPG、LDL-C是Hp感染的危险因素,其中,体检人群中Ⅰ型和Ⅱ型Hp感染率均随着FPG与LDL-C的增高呈上升趋势.但ALB是Ⅰ型Hp感染的保护因素.
Abstract
Objective:To investigate the types of helicobacter pylori(Hp)infection in physical examination population,then analyze the correlation between Hp infection and indicators of fasting plasma glucose(FPG),blood lipids and liver function indexes in adults.Methods:A total of 2 972 adults whose physical examination is accompanied by examination for Hp in Chuzhou Hospital of Anhui Medical University from January 2021 to January 2023 were involved in our study.They were divided into Hp negative group,type Ⅰ Hp group and type Ⅱ Hp group according to types of Hp infection.FPG,blood lipids and liver function were detected simultaneously and the differences between groups were analyzed.Disordered multiple logistic regression was used to analyze the relationship between Hp infection and indicators of fasting plasma glucose,blood lipids and liver function indexes.Results:The results of one-way analysis of variance showed that the levels of FPG and Low-density lipoprotein cholesterol(LDL-C)in type Ⅰ Hp and type Ⅱ Hp groups were significantly higher than those in Hp negative group(P all<0.05).In addition,albumin(ALB)level of type I Hp group was significantly higher than that of Hp negative group(P<0.001);The level of indirect bilirubin(IBIL)in type Ⅱ Hp group was also significantly increased(P<0.05);The level of high-density lipoprotein cholesterol(HDL-C)in type Ⅱ Hp group was significantly decreased(P<0.05).The results of disordered multiple Logistic regression analysis showed that FPG and LDL-C were the risk factors for type Ⅰ Hp compared with Hp negative group.The higher the value of FPG and LDL-C,the more likely they infected with type Ⅰ Hp,ORs were1.253(95%CI 1.163-1.351)and 1.204(95%CI 1.071-1.354),respectively.However,increased ALB level was a protective factor for type ⅠHp,with OR of 0.933(95%CI 0.903-0.963).Meanwhile,FPG and LDL-C were also risk factors for typeⅡ Hp,in the HP negative group,with ORs of 1.205(95%CI 1.091-1.330)and 1.235(95%CI 1.049-1.453),respectively.Conclusion:FPG and LDL-C are risk factors for Hp.The infection rate of type Ⅰ and type ⅡHp increased with the increase of FPG and LDL-C.However,increased ALB is a protective factor for the infection of type Ⅰ Hp.
