5G+AI"三早"慢性病主动健康管理模式的构建与应用评价

慢性病严重威胁人民健康水平的提升,目前学界多方探索通过智能化手段对慢性病进行健康干预与管理.文章基于"三早"主动健康管理理念,在 5G+社区慢性病健康管理系统的基础上引入AI智能助手,通过大数据和数字化技术研发了"'两慢病'患者画像""慢性病管理质控模型""健康评估模块""'两慢病'辅助决策模型"等核心算法模块并应用.实践结果证实,5G+AI"三早"慢性病主动健康管理模式可以对"两慢病"人群进行有效的数据监测管理、数据治理和数据应用,为医防融合服务模式在基层医疗机构推广应用提供科学依据.
Construction and application evaluation of 5G+AI"Three early"chronic disease active health management model
Chronic diseases seriously threaten the improvement of people's health,and the academic community is actively exploring the use of intelligent methods for health intervention and management of chronic diseases.Based on the concept of"three early"active health management,this paper introduces an AI assistant into the 5G+community chronic disease health management system.It has developed core algorithm modules such as"chronic disease patient profiling""chronic disease management quality control model""health assessment module"and"chronic disease assisted decision-making model"through the use of big data and digital technology research and development,and applies them in practice.The practical results confirm that the 5G+AI"three early"chronic disease active health management model can effectively monitor,govern,and apply data for the"two chronic diseases"population,providing a scientific basis for the promotion and application of the medical prevention integration service model in primary medical institutions.
