Objective To explore the postoperative intraocular pressure elevation status in elderly patients with cataract, and analyze its related influencing factors,so as to provide reference for improving the prognosis of patients. Methods The clinical data of 406 elderly patients with cataract were reviewed.According to the occurrence of elevated intraocular pressure at 7 days after surgery, the patients were divided into elevated group(intraocular pressure>21 mmHg)and normal group(intraocular pressure of 10-21 mmHg).The differences in clinical data were compared.Binary Logistic regression analysis was applied to analyze the independent factors affecting postoperative intraocular pressure elevation in elderly patients with cataract. Results There were 35 cases of elevated postoperative intraocular pressure among 406 patients, and the incidence of postoperative intraocular pressure elevation in elderly cataract patients was 8.6%.Univariate analysis showed that there were statistically significant differences in age, surgical duration, intraoperative viscoelastic residue, postoperative intraocular pressure fluctuation, concurrent diabetes mellitus, presence of high myopia and ocular inflammatory reaction between the two groups(P<0.05).Binary Logistic regression analysis found that age(≥75 years old),surgical time(≥3 h),presence of intraoperative viscoelastic residue, postoperative intraocular pressure fluctuation(≥8 mmHg),concurrent diabetes mellitus, presence of high myopia and ocular inflammatory reaction were independent risk factors for postoperative intraocular pressure elevation in elderly patients with cataract(P<0.05).Conclusions There is a certain proportion of elevated intraocular pressure in elderly cataract patients after surgery, which may be affected by age, surgical time, intraoperative viscoelastic residue, postoperative intraocular pressure fluctuation, diabetes mellitus, high myopia, ocular inflammatory reaction and other factors. It is suggested that targeted measures should be taken to prevent postoperative intraocular pressure elevation.
