摘要
目的 探讨经导管动脉化疗栓塞术(TACE)治疗的原发性肝癌患者磷脂酰肌醇-4,5-二磷酸肌醇-3-激酶(PIK3CA)的表达及其与TACE治疗反应的相关性.方法 从TCGA数据库下载425例肝癌患者PIK3CA的表达量.利用GSE104580数据集内TACE治疗敏感和不敏感患者癌组织PIK3CA表达量,分析两组基因表达差异,绘制ROC曲线分析TACE治疗敏感性与PIK3CA表达的相关性.从GSE14520数据集下载具有完整临床资料、接受TACE治疗的肝细胞癌27例,基于PIK3CA的表达量最佳截断值,分成PIK3CA高表达组和PIK3CA低表达组,比较两组患者的临床资料.应用"survminer"R包进行Kaplan-Meier生存分析.结果 肝癌组织PIK3CA表达明显高于癌旁组织;TACE不敏感患者癌组织PIK3CA表达量高于TACE敏感患者,TACE治疗敏感性与PIK3CA表达相关性的ROC曲线下面积(AUC)为0.645;生存分析显示PIK3CA表达量越低,患者生存时间越长,且1、2、3年的AUC分别为0.765、0.713、0.633.结论 PIK3CA对于肝细胞癌有一定的诊断价值,有可能作为TACE治疗敏感性的预测指标.
Abstract
Objective To analyze the expression of phosphatidylinositol-4,5-bisphosphate 3-kinase,catalytic subunit alpha(PIK3CA)in primary liver cancer patients receiving transcatheter arterial chemoemboli-zation(TACE)by using bioinformatics method,and to explore the correlation between the PIK3CA expression level and the therapeutic response to TACE treatment.Methods The PIK3CA expression levels of 425 patients with liver cancer were downloaded from the TCGA database.PIK3CA expression levels in the cancer tissues of sensitive patients and insensitive patients to TACE treatment obtained from GSE104580 data set were used to analyze the gene expression differences between the two groups.ROC curve was drawn to analyze the correlation between TACE treatment sensitivity and PIK3CA expression.A total of 27 patients with hepatocellular carcinoma,who had complete clinical data and received TACE treatment,were downloaded from GSE14520 data set.Based on the optimal cut-off value of PIK3CA expression,the 27 patients were divided into PIK3CA high-expression group and PIK3CA low-expression group.The clinical data were compared between the two groups.The"survminer"R package was used to perform the Kaplan-Meier survival analysis.Results The expression level of PIK3CA within hepatocellular carcinoma tissues was obviously higher than that in the paracancerous tissues.The expression level of PIK3CA within cancer tissues in insensitive patients to TACE treatment was higher than that in sensitive patients to TACE treatment.The area under ROC curve(AUC)for the correlation between TACE treatment sensitivity and PIK3CA expression was 0.645.Survival analysis revealed that the lower the PIK3CA expression level was,the longer the survival time of patients would be,moreover,the one-year,2-year and 3-year AUC values were 0.765,0.713 and 0.633 respectively.Conclusion PIK3CA level carries certain diagnostic value for hepatocellular carcinoma,and probably it may be used as a predictor of sensitivity to TACE treatment.(J Intervent Radiol,2024,33:383-385)
