摘要

肾上腺肿瘤在人群中的发病率约为1%,手术切除是首选治疗方法.近年来许多微创介入技术在肾上腺肿瘤治疗中取得了显著疗效.微波消融术(MWA)以其创伤小、恢复快、可重复性好、安全性高、并发症少等诸多优势,已成功用于肾上腺肿瘤治疗,并得到较广泛认可.因此,MWA是未来肾上腺肿瘤治疗的发展方向之一,可改善局部症状,减轻肿瘤负荷,提高患者生活质量、延长患者生存期.本文就MWA治疗肾上腺肿瘤的原理和机制、导引方式,临床应用于醛固酮瘤、嗜铬细胞瘤、肾上腺皮质腺癌、肾上腺转移瘤的优势和不足,及相关并发症作一综述.

Abstract

The incidence of adrenal tumors in the population is about 1%,and surgical resection has been the first choice of treatment.In recent years,many minimally-invasive interventional techniques have achieved remarkable therapeutic results in the treatment of adrenal tumors.Being of its small trauma,rapid recovery,repeatability,high safety,fewer complications,and many other advantages,microwave ablation(MWA)has been successfully used in the treatment of adrenal tumors and its curative effect has been more widely recognized.Therefore,MWA will become one of the development directions in the treatment of adrenal tumors in the future,it can improve local symptoms,lighten tumor load,improve the quality of life of patients,and prolong the survival time of patients.This article aims to make a comprehensive review of MWA therapy of adrenal tumors,focusing on its principle and mechanism,its guiding methods,as well as its advantages and disadvantages in the treatment of aldosterone tumor,pheochromocytoma,adrenal cortical adenocarcinoma,and adrenal metastases,besides,the MWA-related complications are also discussed.(J Intervent Radiol,2024,33:449-453)
