超声心动图联合腹部、外周血管超声在外源性心脏疾病诊断中的应用

目的 探讨在外源性心脏疾病诊断中应用超声心动图及腹部超声、外周血管超声检查的价值。方法 选取87 例疑似外源性心脏疾病患者,并利用彩色多普勒超声诊断系统进行超声心动图检查,诊断金标准为CT血管造影(CT angiography,CTA)和病理诊断结果。对比超声心动图(简称单项检查)及超声心动图联合腹部、外周血管超声检查(简称联合检查)对心脏结构与功能异常的检出情况。结果 外源性心脏疾病患者有60 例,确诊率达68。97%;单项检查与联合检查诊断外源性心脏疾病的特异性及阳性预测值比较,差异均无统计学意义(P>0。05)。联合检查的诊断敏感性、准确率和阴性预测值明显高于单项检查(P<0。05)。与单项检查相比,联合检查可显著提高心脏结构与功能异常的检出率(P<0。05)。结论 在外源性心脏疾病诊断中联合应用超声心动图及腹部超声、外周血管超声检查,有助于临床准确诊断心脏病变以及外源性病变,可提高确诊率并降低误诊及漏诊风险。联合检查具有更高的诊断效能。
Application of echocardiography combined with abdominal and peripheral vascular ultrasound in exogenous heart disease
Objective To explore the value of echocardiogram,and abdominal and peripheral vascular ultrasound in the diagnosis of exogenous heart diseases.Methods We selected 87 suspected patients with exogenous heart disease who had undergone echocardiogram examination by using a color Doppler ultrasound diagnostic system.The diagnostic gold standard was CT angiography(CTA)and pathological diagnosis results.The detection of cardiac structural and functional abnormalities was compared between echocardiography(single examination),and echocardiography combined with abdominal and peripheral vascular ultrasound(combined examination).Results There were 60 patients diagnosed with exogenous heart disease with a diagnosis rate of 68.97%.There was no statistically significant difference in the specificity and positive predictive value between single and combined examinations in diagnosing exogenous heart disease(P>0.05).The diagnostic sensitivity,accuracy rate,and negative predictive value of the combined examination were significantly higher than those of single examination(P<0.05).Compared with single examination,the combined examination could significantly improve the detection rate of cardiac structural and functional abnormalities(P<0.05).Conclusion The combined application of echocardiography,and abdominal and peripheral vascular ultrasound in diagnosing exogenous heart diseases helps to accurately diagnose cardiac and exogenous lesions in clinical practice.It could increase diagnosis rate,and reduce the risk of misdiagnosis and missed diagnosis.The combined examination has higher diagnostic efficacy.
