目的 探讨基质金属蛋白酶 25(Matrix metalloproteinase 25,MMP25)和E-钙黏蛋白(E-cadherin)对类风湿关节炎的诊断意义.方法 选取 2020 年 1 月至 2023 年 10 月我院 67 例类风湿关节炎患者为实验组,同期 50 例健康体检人群为对照组;采用酶联免疫吸附试验(ELISA)检测二组血清中MMP25 和E-cadherin蛋白水平.收集实验组患者ESR、CRP、CCP、RF、DAS28-ESR、DAS28-CRP指标,以及是否为初诊、是否合并并发症(贫血、间质性肺炎等)、病程、临床活动度等级,并与MMP25 和E-cadherin水平进行相关性分析.结果 实验组血清MMP25 和E-cadherin水平均高于对照组,差异均有统计学意义(P<0.001).与ESR、CRP、CCP和RF等指标进行散点图和简单线性回归性分析,结果显示:不同临床活动度患者表达MMP25 水平差异有统计学意义(P<0.001),MMP25 与病程(β=0.479,P<0.001),MMP25 与CRP(β=0.271,P<0.05),MMP25 与DAS28-ESR(β=0.660,P<0.001)和MMP25 与DAS28-CRP(β=0.624,P<0.001)均有相关性.不同临床活动度患者表达E-cad-herin水平差异有统计学意义(P<0.001),E-cadherin与ESR(β=0.425,P<0.001),E-cadherin与CRP(β=0.328,P<0.05),E-cad-herin与DAS28-ESR(β=0.746,P<0.001),E-cadherin与DAS28-CRP(β=0.671,P<0.001)均有相关性.多重线性回归分析结果显示,MMP25 表达水平与临床活动度正相关(β=1.032,P<0.001),E-cadherin 表达水平与临床活动度正相关(β=0.407,P<0.05)、与病程长短正相关(β=0.497,P<0.001).结论 MMP25 和E-cadherin表达水平可以作为类风湿关节炎评估临床活动度的标志物,此外,E-cadherin表达水平也可以衡量患者病程长短.
Diagnostic value of MMP25 and E-cadherin in rheumatoid arthritis
Objective To investigate the diagnostic significance of Matrix metalloproteinase 25(MMP25)and E-cadherin in rheumatoid arthritis.Methods 67 patients with rheumatoid arthritis in our hospital from January 2020 to October 2023 were cho-sen as the experimental group,and 50 healthy people during the same period were chosen as the control group.The serum levels of MMP25 and E-cadherin were detected by enzyme-linked immunosorbent assay(ELISA).The indexes of ESR,CRP,CCP,RF,DAS28-ESR and DAS28-CRP were collected from patients in the experimental group,as well as whether they were newly diag-nosed,whether they had complications(anemia,interstitial pneumonia,etc),disease course and clinical activity grade and correla-tion with MMP25 and E-cadherin levels were analyzed.Results The serum levels of MMP25 and E-cadherin in rheumatoid arthritis patients were higher than those in control group,with statistical significance(P<0.001).Scatter plot and simple linear re-gression analysis with ESR,CRP,CCP and RF showed that there were significant differences in the expression of MMP25 in differ-ent clinical activity groups(P<0.001),MMP25 and disease course(β=0.479,P<0.001),MMP25 and CRP(β=0.271,P<0.05),MMP25 and DAS28-ESR(β=0.660,P<0.001)and MMP25 and DAS28-CRP(β=0.624,P<0.001)are all correlated;There were significant differences there were significant differences in the expression of E-cadherin in different clinical activity groups(P<0.001),E-cadherin and ESR(β=0.425,P<0.001),E-cadherin and CRP(β=0.328,P<0.05),E-cadherin and DAS28-ESR(β=0.746,P<0.001),E-cadherin and DAS28-CRP(β=0.671,P<0.001).The results of multiple linear regression analysis showed that the expression level of MMP25 was positively correlated with clinical activity(β=1.032,P<0.001),the expression level of E-cadherin was positively correlated with clinical activity(β=0.407,P<0.05),and the duration of disease(β=0.497,P<0.001).Conclusion The expression level of MMP25 and E-cadherin can be used as markers to evaluate clinical activity in rheumatoid arthritis,and the expression level of E-cadherin can also measure the duration of disease.
